Care Team Assistant - Pediatrics (Job Overview)

Schedule: 8AM-5PM

As a Pediatrics Care Team Assistant (CTA), you will be a trusted point of contact and an important informational resource for patients, families and caregivers, the call center, the care team, and community resources. Your compassionate support helps patients navigate their care while enabling providers and clinical team members to deliver high-quality, evidence-based care in accordance with Patient Centered Medical Home (PCMH) requirements.

You will bring together clinical coordination, patient communication, and clerical expertise to help care stay connected and accessible. From supporting phone calls and lab results to assisting with medication management, referrals, pre-appointment planning, and forms, your work will make a meaningful difference in the healthcare experience of the community we serve.

Key Responsibilities

  • Patient Communication: Work with onsite nursing or use protocol-based triage parameters to address patient calls forwarded by the call center and/or front desk.
  • Route clinical inquiries requiring provider review to the responsible provider as a phone note document in the electronic health record system (EHRS).
  • Return patient calls as directed by the provider.
  • Place phone calls or create template-based letters, as directed by the provider, regarding results such as lab results, and deliver them to patients in a timely manner.
  • At the provider’s direction, contact medically vulnerable patients after an office visit for an acute medical problem or patients designated as chronically ill/unstable and in need of ongoing monitoring.
  • Pre-Visit Planning: Review the patient schedule daily to promote completion of pre-visit preparations before each patient visit.
  • Confirm that lab and imaging results, consultation reports, care transition details, and information related to major changes in the patient’s health status are available in the patient record.
  • When information is not present in the EHRS, contact patients, consultants, imaging centers, and/or hospitals to gather it.
  • Report barriers to care to the provider.
  • Assist in arranging interpretation services.
  • Assist with and participate in daily huddles.
  • Care Coordination: Assist with transitions of care, including obtaining hospital and newborn discharge records and specialty notes.
  • Manage and maintain the site’s Care Coordination Desktop.
  • Assist patients with entry to and access of ancillary support services as needed, including prescription assistance, patient education, and social work.
  • Medication Management: Assist with medication refills at the direction of and as instructed by provider(s).
  • Assist with medication prior authorizations as needed.
  • Forms: Initiate completion of forms for provider review and final signature, including disability forms, home health orders, durable medical equipment and supply requests, disease status letters, FMLA paperwork, and other paperwork directed by the provider.
  • Communicate with the provider when urgent requests require prompt attention.
  • Referrals: Assist Referral Coordination staff and onsite scanners with completing referrals and including relevant documents in the medical record.
  • Complete and deliver clinical summaries for consultants or other service providers to whom patients are referred, as directed by the provider or dictated by the service line.
  • Demonstrate respect and regard for the dignity of all patients, families, visitors, and fellow employees to ensure a professional, responsible, and courteous environment.
  • Promote effective working relationships and work effectively as part of a team to facilitate the achievement of goals and objectives.
  • Attend all required safety training programs and describe responsibilities related to general safety, department/service safety, and specific job-related hazards.
  • Attend all required safety education programs.
  • Operate assigned equipment and perform all procedures safely as instructed.
  • Maintain the work area and equipment in the condition required by LCHS standards.
  • Demonstrate proper body mechanics in all functions.
  • Check the patient’s environment and equipment to ensure safety.
  • Report safety concerns promptly to appropriate personnel.
  • Follow LCHS exposure control plans for bloodborne and airborne pathogens.
  • Demonstrate knowledge of techniques, procedures, and correct use of protective barrier equipment, including Universal Precautions.
  • Assure a safe environment by instituting appropriate control measures.
  • Attend annual education programs.
  • Perform other duties as assigned.

Minimum Qualifications

  • High-school level education or equivalency
  • Medical Assistant Certification or its equivalent, or at least 2 years clinical experience in an inpatient or outpatient setting
  • Must maintain BLS /CPR

Required Years of Experience

  • At least 2 years of experience in an ambulatory clinical setting preferred
  • Medical Assistant Certification or its equivalent, or at least 2 years clinical experience in an inpatient or outpatient setting

Required Knowledge, Skills, and/or Abilities

  • Demonstrated competency with an EHRS system
  • Excellent communication skills.
  • Excellent customer service skills.
  • Ability to multi task in a fast paced environment.
  • Above average organizational skills.
  • Strong typing and computer skills.
  • This job requires access to confidential and sensitive information, requiring ongoing discretion and secure information management

Key Responsibilities Patient Communication: Work with onsite nursing or use protocol-based triage parameters to address patient calls forwarded by the call center and/or front desk. Route clinical inquiries requiring provider review to the responsible provider as a phone note document in the electronic health record system (EHRS). Return patient calls as directed by the provider. Place phone calls or create template-based letters, as directed by the provider, regarding results such as lab results, and deliver them to patients in a timely manner. At the provider’s direction, contact medically vulnerable patients after an office visit for an acute medical problem or patients designated as chronically ill/unstable and in need of ongoing monitoring. Pre-Visit Planning: Review the patient schedule daily to promote completion of pre-visit preparations before each patient visit. Confirm that lab and imaging results, consultation reports, care transition details, and information related to major changes in the patient’s health status are available in the patient record. When information is not present in the EHRS, contact patients, consultants, imaging centers, and/or hospitals to gather it. Report barriers to care to the provider. Assist in arranging interpretation services. Assist with and participate in daily huddles. Care Coordination: Assist with transitions of care, including obtaining hospital and newborn discharge records and specialty notes. Manage and maintain the site’s Care Coordination Desktop. Assist patients with entry to and access of ancillary support services as needed, including prescription assistance, patient education, and social work. Medication Management: Assist with medication refills at the direction of and as instructed by provider(s). Assist with medication prior authorizations as needed. Forms: Initiate completion of forms for provider review and final signature, including disability forms, home health orders, durable medical equipment and supply requests, disease status letters, FMLA paperwork, and other paperwork directed by the provider. Communicate with the provider when urgent requests require prompt attention. Referrals: Assist Referral Coordination staff and onsite scanners with completing referrals and including relevant documents in the medical record. Complete and deliver clinical summaries for consultants or other service providers to whom patients are referred, as directed by the provider or dictated by the service line. Demonstrate respect and regard for the dignity of all patients, families, visitors, and fellow employees to ensure a professional, responsible, and courteous environment. Promote effective working relationships and work effectively as part of a team to facilitate the achievement of goals and objectives. Attend all required safety training programs and describe responsibilities related to general safety, department/service safety, and specific job-related hazards. Attend all required safety education programs. Operate assigned equipment and perform all procedures safely as instructed. Maintain the work area and equipment in the condition required by LCHS standards. Demonstrate proper body mechanics in all functions. Check the patient’s environment and equipment to ensure safety. Report safety concerns promptly to appropriate personnel. Follow LCHS exposure control plans for bloodborne and airborne pathogens. Demonstrate knowledge of techniques, procedures, and correct use of protective barrier equipment, including Universal Precautions. Assure a safe environment by instituting appropriate control measures. Attend annual education programs. Perform other duties as assigned. Minimum Qualifications High-school level education or equivalency Medical Assistant Certification or its equivalent, or at least 2 years clinical experience in an inpatient or outpatient setting Must maintain BLS /CPR Required Years of Experience At least 2 years of experience in an ambulatory clinical setting preferred Medical Assistant Certification or its equivalent, or at least 2 years clinical experience in an inpatient or outpatient setting Required Knowledge, Skills, and/or Abilities Spanish fluency (service location dependent) Demonstrated competency with an EHRS system Excellent communication skills.
