Nurse Practitioner - Evansville, WI

At Carease Health - WI, we are committed to delivering exceptional preventative care to nursing home patients through our innovative approach of Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and Collaborative Care Management (CoCM). We are seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) to join our team and make a real difference in patient care.

About the Role:

As a Nurse Practitioner with Carease Health, you will be responsible for delivering high-quality preventative care to residents in skilled nursing facilities. This hybrid role combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation. You will work closely with our experienced clinical team to develop personalized care plans, conduct patients' assessments, and provide medical treatment and education.

Responsibilities:

  • Provide high-quality preventative care to nursing home patients, including on-site visits and remote monitoring

  • Conduct patients' assessments, order diagnostic tests, and develop personalized care plans

  • Collaborate with healthcare teams to improve patient outcomes and reduce hospitalizations

  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care transitions

  • Document patient encounters and treatment plans in a timely and accurate manner

Requirements:

  • Active NP or PA license in the state of practice

  • 1+ year of experience in Geriatric care and documentation review

  • Excellent communication, collaboration, and teamwork skills
