Job Summary:

As a valued member of the Valley Manor team, we are seeking a compassionate and skilled Evening Shift LPN to join our dynamic healthcare team at Valley Manor Rehabilitation and Healthcare Center in Coopersburg, PA. Our state-of-the-art facility offers a wide range of services to patients, and we are committed to placing our patients on the rapid road to recovery. As an Evening Shift LPN, you will play a critical role in providing high-quality patient care and contributing to the success of our interdisciplinary team.

Responsibilities:

  • Provide direct patient care, including administering medications, performing vital signs, and documenting patient information

  • Develop and implement individualized care plans in collaboration with the interdisciplinary team

  • Assist with admissions, discharges, and transfers, ensuring a seamless transition for patients and families

  • Collaborate with physicians and other healthcare professionals to address patient needs and concerns

  • Maintain accurate and detailed records of patient care and treatment

  • Provide education and support to patients and families to promote independence and self-management

  • Participate in quality improvement initiatives to optimize patient outcomes and satisfaction

Requirements:

  • Current LPN licensure in the state of Pennsylvania

  • High school diploma or equivalent required; currently enrolled in an LPN program or has already completed a LPN program with a minimum of [insert minimum years of experience]

  • Excellent communication and interpersonal skills
