Responsibilities:

  • Provide direct patient care to residents, including administering medications, taking vital signs, and performing wound care, under the supervision of a licensed RN or MD

  • Develop and implement individualized care plans, in collaboration with the interdisciplinary team, to meet the unique needs of each resident

  • Maintain accurate and up-to-date records of patient care, including medication administration, treatments, and test results

  • Monitor and report any changes in patient condition, including new allergies, infections, or injuries

  • Collaborate with other healthcare professionals to ensure effective communication and coordination of care

  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction

  • Assume responsibility for maintaining a clean and safe environment, adhering to infection control protocols and following facility policies and procedures
