About the Role:

As an LPN at The Gardens at Country, you will be responsible for delivering direct patient care, working collaboratively with our interdisciplinary team, and ensuring that our residents receive comprehensive and individualized care. Some of your key responsibilities will include:

• Providing high-quality patient care: Administer medications, perform vital signs, and monitor medical conditions, while maintaining open communication with our healthcare team.
• Assisting with daily living activities: Support residents with personal care, mobility, and daily living tasks, promoting independence and dignity.
• Conducting routine assessments: Monitor residents' conditions, identifying potential concerns and reporting changes to our healthcare team.
• Documenting patient information: Accurately record patient data, medication administration, and treatment plans in our electronic medical record system.
• Participating in quality improvement initiatives: Collaborate with our team to identify areas for improvement, implement changes, and monitor outcomes.
