Certified Nursing Assistant (CNA)
Pine Valley Center for Rehabilitation & Nursing is seeking compassionate, dependable, and hardworking Certified Nursing Assistants (CNAs) to join our nursing team!
We are a 160-bed skilled nursing and rehabilitation facility dedicated to providing high-quality, resident-centered care in a supportive and professional environment.
We are looking for CNAs who take pride in their work, demonstrate compassion for our residents, and understand the importance of teamwork, accountability, and attention to detail.
Our ideal candidate is proactive, attentive to resident needs, and committed to providing quality care—not simply completing assigned tasks.

Perform regular resident rounding throughout the shift and respond promptly to call bells.
Follow established turning and repositioning schedules, including the facility's two-hour repositioning program for residents requiring pressure injury prevention interventions.
Monitor residents for skin changes, discomfort, unusual behaviors, or changes in condition and report concerns promptly to the nurse.
Maintain a safe, clean, and organized resident environment.
Ensure call bells, personal belongings, and essential items remain within residents' reach.
Follow fall prevention measures, infection control precautions, and individualized safety interventions.
Assist with appropriate use of pressure-relieving devices and positioning equipment as directed by the nursing team.
Documentation & Assigned Duties
Complete assigned CNA documentation accurately and promptly in the electronic medical record.
Record and report intake and output, bowel movements, meal consumption, toileting, and other required observations.
Obtain and document vital signs and resident weights as assigned and within the CNA's scope of practice.
Complete scheduled showers, weights, and other assigned responsibilities during the designated shift.
Communicate incomplete tasks or changes in resident condition to the assigned nurse before the end of the shift.
Participate in shift-to-shift reporting to promote continuity of care.
Teamwork & Professional Accountability
Work collaboratively with nurses, fellow CNAs, rehabilitation staff, and other departments.
Follow assignments and instructions provided by the Charge Nurse, RN Supervisor, Unit Manager, and nursing administration.
Remain attentive and available to assigned residents throughout the shift.
Demonstrate professional behavior, punctuality, reliability, and respectful communication.
Assist coworkers when needed to ensure residents receive timely care.
Maintain appropriate professional boundaries and follow facility policies regarding personal cell phone use and resident confidentiality.
Report resident concerns, incidents, and potential safety issues immediately.
Participate in required in-services, education, and competency training.
