A Place at Home Encinitas

Geriatric LVN Care Supervisor Part Time

$33 - $36 / hour

A Place at Home – Encinitas provides personalized, non-medical home care that helps older adults remain safe, supported, and independent at home. We’re hiring a non-clinical LVN/Care Supervisor to train and supervise caregivers, coordinate senior care resources, and support clients and families as we grow, ensuring consistent care and dependable follow-through.

This is a non-clinical position with no hands-on nursing care. The focus is geriatric care management, resource navigation, caregiver ADL training/ supervision, and quality assurance.

Independence and Initiative Required. You must be a self-starter, independent worker, and resourceful problem solver with exceptional attention to detail. We provide onboarding, clear procedures, and leadership support. You are expected to manage routine responsibilities without repeated reminders or step-by-step direction.

For example, if a client struggles to obtain meals, you would explore the barriers, help arrange an appropriate meal delivery service with authorization, and follow up to confirm it is working. You take ownership while recognizing when approval, clinical guidance, or escalation is needed.

Responsibilities:

  • Assess non-medical needs and home safety; maintain individualized care plans.

  • Coordinate new-client starts, caregiver handoffs, and follow-up visits.

  • Train caregivers on ADLs and transfer techniques--observe competency, provide coaching, and verify improvement.

  • Review care logs, monitor client satisfaction, and resolve service concerns.

  • Help clients and families navigate resources and coordinate authorized referrals.

  • Partner with scheduling to support caregiver matching and continuity of care.

  • Maintain timely documentation, communicate outcomes, and consistently follow our Care Management SOP.

Senior Care Resource Knowledge—Required:

You must thoroughly understand the senior care ecosystem and know how to identify resources, explain options, coordinate referrals, and verify that services are in place. This includes:

  • In-home medication support, medication delivery, and mail-order pharmacies.

  • Home health, hospice, and palliative care.

  • Durable medical equipment (DME) and assistive devices.

  • Medicare, Medi-Cal, private insurance, and long-term care insurance resources.

  • Veterans Administration (VA) programs and benefits.

  • Home modifications, accessibility, and fall-prevention resources.

  • Independent living, assisted living, memory care, and other senior housing.

  • Meal delivery, transportation, respite, and community support programs.

You should understand when provider orders, eligibility reviews, or insurance authorization may be needed. San Diego County resource familiarity is strongly preferred.

Required Qualifications

  • Active California LVN license in good standing.

  • Three or more years of geriatric or elder care experience.

  • Strong judgment, organization, communication, documentation, and caregiver coaching skills.

  • Comfort using electronic records, email, and mobile technology.

  • Valid California driver’s license, good driving record, reliable vehicle, and current automobile insurance meeting or exceeding state minimum requirements.

  • Ability to perform essential duties with or without reasonable accommodation.

Schedule—Required Availability:

Monday–Friday, 8:30 a.m.–2:00 p.m., totaling 27.5 regularly scheduled hours per week.

Rotating evening and weekend coverage is REQUIRED approximately 15% of each month, primarily for caregiver handoffs, training, and evening new-client starts. These assignments may involve adjusted or additional hours.

Work Location and Required Travel:

Work is split between our main office in Encinitas, CA 92024, and client homes.

Driving to clients’ homes is required, primarily Oceanside south to La Jolla and inland to Poway. Candidates must be comfortable traveling throughout this service area.

Pay and Benefits:

  • $33–$36/hour, depending on relevant experience and supervisory qualifications.

  • Paid time off (PTO) and paid sick leave.

  • Free wellness program

  • Eight designated federal holidays off each year

  • Eligible business mileage reimbursement at the applicable IRS rate
