Responsibilities of the Intake RN - CHHA:

  • Review incoming clinical records and patient histories to evaluate medical necessity and clinical appropriateness for home health services

  • Process initial physician orders and verify mandatory documentation according to state and federal regulations

  • Build preliminary care plans and assign visiting disciplines based on patient acuity and needs

  • Act as the primary liaison between hospitals, discharge planners, physicians, internal clinical teams, and the patient's family

  • Explain what to expect from home care services, answer initial questions, and provide guidance on patient rights and agency processes

Qualifications of the Intake RN - CHHA:

  • Must have current New York State RN license

  • Must have at least two years of experience in a CHHA intake department or MLTC intake all scripts experience

  • Must have a minimum of one year supervisory experience

  • Must have excellent clinical, writing and communication skills
