About the Role:

As a Licensed Practical Nurse (LPN) at Autumn Lake Healthcare at Pikesville, you will be responsible for:

  • Providing direct patient care to our residents, including administering medications, performing wound care, and conducting vital signs monitoring

  • Developing and implementing individualized care plans in collaboration with the interdisciplinary care team

  • Communicating effectively with residents, families, and healthcare professionals to ensure seamless care transitions

  • Participating in quality improvement initiatives to enhance resident outcomes and satisfaction

Requirements:
