Job Responsibilities

  • Working as an LPN supporting the diverse needs of residents in post-acute facilities
  • Monitoring residents' mental and physical conditions and vital signs
  • Facilitating the intake process, including the documentation of medical histories and any health issues
  • Assisting with medication administration, including the verification process and the monitoring of effects on residents
  • Maintaining residents' medical records and charts
  • Helping with immunizations, IVs, and blood draws
  • Ensuring residents have a comfortable and safe environment
  • Being accountable to adhere to residents' care plans and to comply with all applicable rules and regulations
  • Contributing to the maintenance and proper inventory of medical supplies
  • Providing assistance with new hire orientation and training

Qualifications and Skills

  • Current Massachusetts state license to practice as a Licensed Practical Nurse (LPN)
  • Completion of approved Licensed Practical Nurse (LPN) educational program
  • High school diploma or GED (or equivalent)
  • 6 months experience as a LPN (or similar role)
  • Ability to lift 50 lbs

Per-Diem LPN - Greater Boston/ Boston, MA Always Best Care Private Nursing is seeking an experienced LPN to assume responsibility and accountability for the application of the nursing process and the delivery of one-to-one patient care, all in the comfort of the client’s home. This role requires someone who demonstrates the ability to make clinical judgments in an effective and efficient manner under the direction of the Director of Private Nursing or RN supervisor. Role and Responsibilities: LPNs assist in delivering nursing care under the supervision of an RN, including contributing to patient assessments, implementing components of care plans developed by an RN, administering medications, performing skilled nursing tasks (e.g., wound care), and reinforcing patient and family education. They may assist in the oversight of CNAs, QMAPs, and non-certified personnel, within their scope and under RN direction. Responsibilities include: Contributes to the ongoing assessment of the client’s health status and reports findings to the RN. Implements and documents nursing interventions based on the plan of care developed by an RN. Reports signs and symptoms indicating physiological or psychosocial changes to the supervising RN promptly. Administers medications and treatments as prescribed and authorized by an RN. Reinforces teaching provided by RNs and providers, and offers guidance to patients and families consistent with established care plans. Participates in collaboration with the care team to promote continuity of care. Performs delegated tasks such as wound care, catheter care, and injection administration within LPN scope and based on individual training. Communicates clearly and promptly with the RN or Director of Nursing regarding changes in condition or care needs. Other duties as assigned, within the LPN scope and under RN supervision.
