Job Responsibilities

  • Working as an LPN supporting the diverse needs of residents in post-acute facilities
  • Monitoring residents' mental and physical conditions and vital signs
  • Facilitating the intake process, including the documentation of medical histories and any health issues
  • Assisting with medication administration, including the verification process and the monitoring of effects on residents
  • Maintaining residents' medical records and charts
  • Helping with immunizations, IVs, and blood draws

The SE Licensed Practical Nurse will work closely with their respective supervisor as well as the other nursing staff to coordinate all care for SE participants. Assignments will require occasional or frequent travel throughout the service area to/from participants’ homes and contracted facilities.
