Job Responsibilities

  • Working in post-acute facilities in the area as an LPN
  • Being accountable to follow and adhere to resident care plans
  • Tracking the wellbeing of residents, both physical and mental, and monitoring their vital signs
  • Conducting intakes, documenting health concerns and medical history of residents
  • Administering and properly verifying medications and monitoring their effects
  • Reviewing residents' charts and medical records
  • Assisting with the cultivation of a safe, comfortable facility
  • Maintaining appropriate stocks of medical supplies
  • Participating in IV set up, immunizations, and phlebotomy procedures
  • Helping with the orientation and training of medical staff

Qualifications and Skills

  • Current Ohio state license to practice as a Licensed Practical Nurse (LPN)
  • Completion of approved Licensed Practical Nurse (LPN) educational program
  • High school graduation, GED, or equivalent
  • Degree from an approved LPN training program
  • At least 6 months of experience (LPN or similar role)
  • Ability to lift a minimum of 50 lbs

Makes written and verbal recommendations to the Director of Clinical Services concerning the operations of the nursing department. Participates in associate evaluations and associate disciplinary action as required. Ensures proper transition of nursing reports upon starting and ending shift duty hours.
