Nurse Anesthetists administer general, regional and monitored anesthesia care for scheduled and emergency surgical, obstetric and diagnostic procedures. In addition to providing excellent care, our CRNA’s also provide one-on-one anesthesia instruction for Columbia University and Fairfield University’s Nurse Anesthesia Program. In April of 2018, we opened the state-of-the-art David H. Koch Ambulatory Surgery Center that has become the focal point of ambulatory surgery at New York-Presbyterian Hospital. In 2020, we expanded the DHK Ambulatory Center to house the Steven & Alexandra Cohen Women’s Hospital. Description The certified registered nurse anesthetist is an advanced practice nurse (CRNA). Within this role, under the supervision of an Attending Anesthesiologist, the CRNA, administers, monitors and manages patient care before, during and after anesthesia. Responsibilities Clinical Practice 1. In conjunction with Attending Anesthesiologist, performs a thorough and completed pre-anesthesia assessment. Responsible for obtaining and reviewing relevant tests and discussing findings with the Attending. 2. Obtains and/or verifies that an informed consent has been obtained for the planned anesthetic intervention from the patient or legal guardian. Discusses anesthetic options and risks with the patient and/or legal guardian after discussion with Attending Anesthesiologist. Documents in the patients medical record that informed consent was obtained. 3. Formulates a patient-specific plan for anesthesia care in conjunction with the Anesthesiologist. The plan is based upon comprehensive patient assessment, problem analysis, anticipated surgical or therapeutic procedure, patient and surgeon preferences, and current anesthesia principles. 4. Induces anesthesia under the supervision of an Attending Anesthesiologist and maintains anesthesia at required levels. Assesses continuously the patients response to the anesthetic and/or surgical intervention and must immediately notify the Attending Anesthesiologist of significant changes in the patients condition and intervenes as required, under the supervision of an Attending Anesthesiologist to maintain the patient in a satisfactory physiologic condition. 5. Monitors the patients physiologic condition as appropriate for the type of anesthesia and specific patient needs. * Monitors ventilation continuously. Verifies intubation of the trachea by auscultation, chest excursion, and confirmation of carbon dioxide in the expired gas. Continuously monitors end-tidal carbon dioxide during controlled or assisted ventilation. Uses spirometry and ventilatory pressure monitors. * Monitors oxygenation continuously by clinical observation, pulse oximetry, and if indicated, arterial blood gas analysis. * Monitors cardiovascular status continuously via electrocardiogram and when appropriate, heart sounds. * Records blood pressure and heart rate. * Monitors body temperature continuously on all pediatric patients receiving general anesthesia and when indicated, on all other patients. * Monitors neuromuscular function and status when neuromuscular blocking agents are administered. * Monitors and assesses the patient positioning and protective measures. 6. The omission of any monitoring standards shall be documented, and the reason stated on the patients anesthesia record. The CRNA shall be in constant attendance of the patient until another qualified health care provider has accepted the responsibility for care. 7. Maintains complete, accurate, and timely documentation of pertinent information on the patients medical record. Must document all anesthetic interventions and patient responses. 8. Transfers the responsibility for care of the patient to other qualified providers in a manner that assures continuity of care and patient safety. 9. Adheres to appropriate safety precautions, as established within the institution, to minimize the risk of fire, explosion, electrical shock and equipment malfunction. Documents on the patients medical record that the anesthesia machine and equipment were checked. 10. Inspects the anesthesia machine prior to use and monitors according to established guidelines. Checks the readiness, availability, cleanliness, and working condition of all equipment to be utilized in the administration of the anesthesia care. When the patient is ventilated by an automatic mechanical ventilator, monitors the integrity of the breathing system with a device capable of detecting a disconnection by emitting an audible alarm. Monitors oxygen concentration continuously with an oxygen supply failure alarm system. 11. Takes precautions to minimize the risk of infection to the patient, the Nurse Anesthetist, and other healthcare providers. Professional Practice 1. Participates in the ongoing review and evaluation of the quality and appropriateness of anesthesia care as a member of the Department of Anesthesiology. Evaluation shall be performed based upon appropriated outcome criteria and reviewed on an ongoing basis. 2. Participates in a continual process of self-evaluation and strives to incorporate new techniques and knowledge into practice. 3. Participates in departmental Grand Rounds, presenting relevant cases as appropriate. 4. Respects and maintains the basic rights of patients. 5. Assists in research projects as directed by an anesthesiologist. 6. Participates in the instruction and supervision of Student Registered Nurse Anesthetists (SRNAs). Administrative Practice 1. Within this role, under the supervision of an Attending Anesthesiologist, the CRNA, administers monitors and manages patient care before, during and after anesthesia. Responsibilities Responsible To Designated Attending Anesthesiologists/ Vice President, Nursing and Chief Certified Registered Nurse Anesthetist for performance appraisals and scheduling Responsible For The performance of pre-anesthesia assessment, induction and maintenance of and emergence from anesthesia, monitoring of the patient's physiological condition and performance of clinical intervention as necessary, under the supervision of an Attending Anesthesiologist. Principal Duties and Responsibilities Clinical Practice 1. In conjunction with Attending Anesthesiologist, performs a thorough and completed pre-anesthesia assessment. Responsible for obtaining and reviewing relevant tests and discussing findings with the Attending. 2. Obtains and/or verifies that an informed consent has been obtained for the planned anesthetic intervention from the patient or legal guardian. Discusses anesthetic options and risks with the patient and/or legal guardian after discussion with Attending Anesthesiologist. Documents in the patients' medical record that informed consent was obtained. 3. Formulates a patient-specific plan for anesthesia care in conjunction with the Anesthesiologist. The plan is based upon comprehensive patient assessment, problem analysis, anticipated surgical or therapeutic procedure, patient and surgeon preferences, and current anesthesia principles. 4. Induces anesthesia under the supervision of an Attending Anesthesiologist and maintains anesthesia at required levels. Assesses continuously the patient's response to the anesthetic and/or surgical intervention and must immediately notify the Attending Anesthesiologist of significant changes in the patient's condition and intervenes as required, under the supervision of an Attending Anesthesiologist to maintain the patient in a satisfactory physiologic condition. 5. Monitors the patient's physiologic condition as appropriate for the type of anesthesia and specific patient needs. * Monitors ventilation continuously. Verifies intubation of the trachea by auscultation, chest excursion, and confirmation of carbon dioxide in the expired gas. Continuously monitors end-tidal carbon dioxide during controlled or assisted ventilation. Uses spirometry and ventilatory pressure monitors. * Monitors oxygenation continuously by clinical observation, pulse oximetry, and if indicated, arterial blood gas analysis. * Monitors cardiovascular status continuously via electrocardiogram and when appropriate, heart sounds. * Records blood pressure and heart rate. * Monitors body temperature continuously on all pediatric patients receiving general anesthesia and when indicated, on all other patients. * Monitors neuromuscular function and status when neuromuscular blocking agents are administered. * Monitors and assesses the patient positioning and protective measures. The omission of any monitoring standards shall be documented and the reason stated on the patients anesthesia record. The CRNA shall be in constant attendance of the patient until another qualified health care provider has accepted the responsibility for care. 6. Maintains complete, accurate, and timely documentation of pertinent information on the patients' medical record. Must document all anesthetic interventions and patient responses. 7. Transfers the responsibility for care of the patient to other qualified providers in a manner that assures continuity of care and patient safety. 8. PART I. JOB SPECIFIC DUTIES 55% Conducts and maintains the anesthetic as evidenced by: Selects and implements appropriate anesthetic agent. Initiates and maintains patient ventilation when appropriate. Provides for protection of the patient's eyes and pressure points when appropriate. Observes and records the effects of ambient temperature on the patient when appropriate. Evaluates the adequacy of anesthetic depth. Evaluates, records, and treats any cardiovascular dysrhythmia. Evaluates, manages, and documents fluid and electrolyte status. Identifies and determines the need for blood component therapy. Effectively adjusts the anesthetic to meet requirements of patient need. Evaluates the need for drug antagonism and reversal. Recognizes and reports any abnormal patient responses. 20% Administers/Induces Anesthesia as evidenced by: Provides an appropriate general anesthetic (i.e. rapid sequence) Establishes and maintains patient airway with OETT, while minimizing laryngoscopy time, establishing tube placement within 30 seconds, observes and affirms tube placement (breath sounds, ETCO2 level, capnograph pattern). Establishes regional anesthesia in a safe and timely manner. 5% Completes and documents pre-anesthesia patient assessment according to current anesthesia standards for any person requiring any form of anesthesia or IV sedation as evidenced by: Collaborates with all members of the health care team to ascertain acuity needs of patient. Chart review, patient interview, and appropriate documentation of patient history. Physical examination of patient, including airway examination. Offers psychological support while explaining the anesthetic risk/benefit and obtaining an informed consent for all planned and potential forms of anesthesia. 5% Selects appropriate equipment and techniques for use in the accomplishment of goals outlined in the anesthesia care plan/management plan as evidenced by: Evaluation of the functional status of anesthesia equipment (machine, suction, laryngoscope, monitors, etc.) Assures presence and functioning of emergency equipment prior to initiation of any anesthetic technique. Calculation of drug doses; preparation and appropriate labeling of syringes. Appropriate selection of airway management equipment. Appropriate application and selection of monitoring modalities. 5% Supervises Student Registered Nurse Anesthetists (SRNAs) in the delivery of anesthesia care. Participates in education program for SRNAs. 2% Appropriately and effectively administers pharmacological agents. Administers drugs based on individual patient need, dosage and interaction. Follows hospital policy on controlled substances security and sign out. Evaluates patient response to pharmacological agents. Effectively treats any adverse drug reaction. 2% Terminates the anesthetic and emerges the patient from anesthesia. Prepares the patient for emergence. Demonstrates knowledge of extubation criteria. Extubates patient and evaluates ventilatory status. Determines patient stability prior to transport to PACU. Leaves the OR in a state of readiness prior to leaving for PACU. Transports patient safely to PACU. Gives a thorough report to the PACU nurse. 2% Evaluates the attainment of anesthesia care goals. Evaluates response to anesthesia care (immediate or delayed). Evaluates and documents patient status on admission to PACU. Recognizes current or possible problems/complications/side effects. Communicates evaluation findings with the pertinent members of the health care team. 2% Completes departmental documentation and post anesthesia rounds. Completes patient care log appropriately. Completes narcotic record appropriately. Completes and documents post-anesthesia rounds. 2% Assists with starting difficult intravenous lines, inserting difficult arterial lines, obtaining difficult arterial blood gases, performing emergency endotracheal intubation and cardiopulmonary resuscitation throughout the hospital. Graduation with a minimum of a master's degree from a nurse anesthesia program accredited by the Council on Accreditation of the American Association of Nurse Anesthetists. Pass the national certification examination within 24 months after graduation. License/Certification Required: Current New York State Registered Nurse's License.
