Our home health aides go beyond taking vital signs and administering medicine. They also provide personal hygiene assistance, help with small things around the house, pray with our patients, and prepare light meals or snacks – all while accurately assessing the patient’s status and communicating it back to the supervising clinician. Records intake, output, and vital signs as directed and communicates care as appropriate. Provides assistance with personal hygiene and elimination needs according to the established plan of care. Assists patients with ambulation and transfer using proper body mechanics. Communicates any change in condition or lack of patient progress. Schedules visits with the patient or caregiver. Observes residents and reports to nursing any changes in physical, mental, and emotional condition. Communicates and interacts in a professional, respectful, and hospitable manner. Qualifications Certified Nursing Assistant (CNA) or Home Health Aide (HHA) certification required. Ability to prioritize and organize work effectively and efficiently. Ability to read, write, speak & understand the English language. Must have no finding in the registry concerning abuse, neglect, or misappropriation of resident property. Can foster good resident and family relationships. Ability to effectively collaborate within a team environment. A passion for working with elderly residents. Experience in a long-term or assisted living facility a plus, but not required.
