Job Summary Acute Rehab Unit Registered Nurse (Night Shift) provides direct nursing care to patients with temporary, progressive or permanent physical disabilities, trauma and or chronic illness. Working together with a multidisciplinary team, the Rehab nurse through careful assessment creates and implements a plan of care, and interventions to assist patients in adapting to an altered lifestyle, and to attain and maintain maximum function while providing a therapeutic environment for both the patients and their family members development. Schedule: 2,12-hour shifts, 5pm-5:30am. Job Responsibilities The Staff Nurse, Acute inpatient Rehab services, is a Nurse that provides direct nursing care to patients with temporary, progressive or permanent physical disabilities, trauma and or chronic illness. Create and implement a written plan of care that addresses the specific needs and goals of each individual patient, making referrals to other professional services as required. Modifies the plan of care based on reassessment of the patient including observed patient response to treatment/care and evaluation of patient's progress toward goal completion and delegates tasks to team members, as appropriate within their scope of practice. Observe, document, address, and implement planned interventions/care, communicating the plan to all members of the team through accurate and timely documentation in the electronic medical record and, when needed, through verbal reports. Documents accurate patient information in compliance with policies and regulatory requirements. Provide patient care through the performance of selective procedures that include but are not limited to IV starts and restarts, blood administration and phlebotomy tasks, answering call lights and serving as testing personnel under CLIA1988 regulations as they relate to point of care testing. Administer medications, accurately and timely according to physician orders and organizational policy and procedure. Maintains knowledge of medication actions and side effects, recognizing allergic reactions and documenting patients’ response to drugs. Successfully transition patients, communicating the most current plan of care, through walking rounds and providing accurate, timely report to incoming staff. Advise physicians, other disciplines, family/significant others of changes in patient's condition or abnormal findings. Communicate clearly and concisely. Provide clear, specific educational and discharge information to each patient and/or patient family member. May precept new employees and oversee other members of the care team. Job Summary The Registered Nurse, Emergency Department is responsible for handling initial patient assessments, performing laboratory work, point of care testing and administrative tasks associated with the delivery of patient care. The Emergency Department Registered Nurse collaborates with physicians regarding discharge plans, creates and educates patients upon discharge from the Emergency Department and works effectively in transferring patients who are admitted to the hospital. Job Responsibilities Triages patients who present to the Emergency Department, prioritizing care based on the critical nature and severity of a patient’s condition. Assesses patient condition through visual evaluation, medication reconciliation, and vital sign measurements such as: blood pressure reading pulse rate, respiration, and temperature throughout the course of a patient’s stay in the Emergency Department. Administers medication and assists with medical procedures as prescribed by the treating physician. Monitors patients for changes in condition and communicates timely and effectively with those involved in the welfare, care or treatment of the patient in compliance with applicable policies and regulations. Partners with other members of the patient care treatment team in the provision of procedures, interventions, discharge or admission processes. Charts all patient medical history, contact information, condition, medications, and treatment as well as update the electronic medical record throughout the patient stay in the Emergency Department. Charting is accurate and consistent with applicable department and organization standards. Discharges patients from the Emergency Room post treatment. Completes discharge instructions, educates the patient and/or caregiver ensuring understanding of the instructions, answers questions, and confirms transportation from the Emergency Department. Attend continuing education classes when required. Maintains required licenses. Attend 75% of all scheduled department meetings. Comply with all applicable policies and procedures, including, but not limited to all departmental and functional policy and procedure. Participate in performance improvement activities as requested. SUMMARY: The primary purpose of this job position is to assist in the planning, implementation, and evaluation of recreational, social, intellectual, emotional and spiritual programs, in accordance with the resident’s assessment and care plan, and as may be directed by your supervisors. ESSENTIAL DUTIES AND RESPONSIBILITIES: Assist in planning, developing, organizing, implementing & evaluating the activity programs & quality assurance program in this facility for the Activities Department. Observe resident attendance, mood, behavior, and degree of involvement so that the facility activities & resident progress are evaluated, by noting, reporting and charting resident behavior. Assume the authority, responsibility & accountability of Activity Assistant/Aide. Ensure that all charted activity progress notes are informative and descriptive of the services provided and indicate the resident’s response to the service. Develop & maintain a good working rapport with other departments within the facility to assure that daily activities can be performed without interruption. Create & maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the facility. Assist in preparing activity rooms/areas for scheduled activity programs. Recommend to your supervisor the equipment and supply needs of the activity department. Assist in cleaning up activity areas after completing activity functions. Follow the care plan in providing daily activities for resident. Purchase department supplies, clothing and personal items for the residents. Maintain the confidentiality of all resident care information including protected health information & follow facility guidelines in reporting non-compliance with said guidelines & treat residents with dignity at all times. Oversee residents engaged in scheduled activities to promote the physical, social, and mental well-being of residents by applying experience in activities and knowledge of diagnoses and conditions of residents. Assists in-room residents by visiting with them, writing letters, running errands, and perform assistance with crafts, projects, etc., as necessary. QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements below are representative of the knowledge, skill, and/or ability required. Experience in Long Term Care Activities Department is preferred. Must have patience, tact, cheerful disposition & enthusiasm, as well as be willing to handle residents based on whatever maturity level at which they are currently functioning. Must be able to read, write, speak, and understand the English language.
