Registered Nurse Resource Team - Nights

Job Summary The Resource Pool Registered Nurse provides direct nursing care to patients through assessment, the creation of individualized plans of care, and implementation, coordination, and delivery of care within the assigned unit that is aligned with the delivery of exceptional patient care. Job Responsibilities Create and implement a written plan of care that addresses the specific needs and goals of each individual patient, making referrals to other professional services as required. Modify the plan of care based on reassessment of the patient including observed patient response to treatment/care and evaluation of patient's progress toward goal completion and delegates tasks to team members, as appropriate within their scope of practice. Observe, document, address, and implement planned interventions/care, communicating the plan to all members of the team through accurate and timely documentation in the electronic medical record and, when needed, through verbal reports. Document accurate patient information in compliance with policies and regulatory requirements. Provide patient care through the performance of selective procedures that include but are not limited to IV starts and restarts, blood administration and phlebotomy tasks, answering call lights and serving as testing personnel under CLIA1988 regulations as they relate to point of care testing. Administer medications accurately and timely according to physician orders and organizational policy and procedure. Maintain knowledge of medication actions and side effects, recognizing allergic reactions and documenting patients’ response to drugs. Successfully transition patients, communicating the most current plan of care, through walking rounds and providing accurate, timely report to incoming staff. Advise physicians, other disciplines, family/significant others of changes in patient condition or abnormal findings. Communicate clearly and concisely. Provide clear, specific educational and discharge information to each patient and/or patient family member. May precept new employees and oversee other members of the care team. Adhere to the agreed upon shift and unit scheduling as defined in the Resource Pool Position Guidelines. Other duties as assigned. Education Requirements Minimum: Associate degree from an accredited school of nursing Preferred: Bachelor degree from an accredited school of nursing Experience Requirements Minimum: one (1) year as a Registered Nurse in a hospital setting in each of the covered areas. Job Summary Medical/Surgical/Telemetry nursing is the core of adult inpatient nursing care and this group of nurses is multi-talented. The Registered Nurse in the Medical and Post-Surgical Units at Riverview Health provides direct nursing care to patients with acute or service specialty diagnoses such as Cardiology, Neurology, Endocrinology, Urology, Renal, Pulmonary, Infectious Disease, inpatient Hospice care. Nurses on this unit may also be assigned post-operative surgical orthopedic, abdominal, general surgery, ENT, and gynecology patients. A hospitalist team is available 24/7 to answer questions, teach and support the nursing team. Medical and Surgical Nurse Navigators act as liaison with providers, in a team-oriented, patient-focused environment. On-the-unit orientation, ongoing education and resources support skill development and mastery. Job Responsibilities Triages patients who present to the Emergency Department, prioritizing care based on the critical nature and severity of a patient’s condition. Assesses patient condition through visual evaluation, medication reconciliation, and vital sign measurements such as: blood pressure reading pulse rate, respiration, and temperature throughout the course of a patient’s stay in the Emergency Department. Administers medication and assists with medical procedures as prescribed by the treating physician. Monitors patients for changes in condition and communicates timely and effectively with those involved in the welfare, care or treatment of the patient in compliance with applicable policies and regulations. Partners with other members of the patient care treatment team in the provision of procedures, interventions, discharge or admission processes. Charts all patient medical history, contact information, condition, medications, and treatment as well as update the electronic medical record throughout the patient stay in the Emergency Department. Charting is accurate and consistent with applicable department and organization standards. Discharges patients from the Emergency Room post treatment. Job Summary The Wound Care Nurse provides advanced wound care in collaboration with members of the care team. The Nurse assesses wounds and implements a care plan designed towards optimal wound care outcomes. Job Responsibilities Assess and monitor wounds. Clean and bandage as required. Implement the wound/ostomy/continence treatment plan of care. Explain procedures, medications, and treatment to be administered to patient and authorized friend or family member.
