Activity contributes to team effort by helping, supporting and being present/available on the floor. · Plans, organizes and directs activities of all unit staff available on the shift. · Leads during complex/stressful situations. Ensures timely and appropriate actions and follows up on the effectiveness of the solution. · Maintains a cooperative relationship among health care teams by communicating relevant and timely information. · Schedule: Timely prepares and communicates the units’ monthly schedule, balancing patient acuity, census, staff skills, experience availability and special requests to minimize overtime use and to meet budgetary needs. · Staff availability: identifies staffing patterns to cover the units’ needs around the clock, performs daily adjustment to schedule, and proactively calls team members if the operation needs help. · Payroll: verifies and updates payroll system on a daily basis and troubleshoots with Director and pending issue. · Process confirmation: Executes all processes confirmations scheduled for the shift. Provides immediate feedback to employee (strength and opportunity areas), updates tracking too and adds comments. · Incident Reporting: timely reports incidents and discusses with Director Opportunities and solutions to prevent incidents from reoccurring. · Licenses and accreditations: Ensures All Nurses have all appropriate licenses/accreditations, keeping track and anticipating due dates. · Interviews potential candidates and provides new employee onboarding is effective. · Provides continuous and effective coaching and continuously monitors team member’s performance and does appraisal meetings twice a year. · Ensures correct operation and availability of medical and administrative equipment by tracking inventories and location. · Ensures infection-control policies and protocols, medication administration, storage procedures and controlled substance regulations. · Ensure compliance safety, clinical/operational processes, procedures and policies, state board of nursing and state nurse practice act requirements. · Continuously obtains updated information about the Key Performance indicators chosen by leadership for the unit. · Promotes a healthy work environment. · All other tasks as related to the position’s duties. Including clinic nurses, home care and UR nurse, medical assistants and other technicians.. · Perform rounds in Eldercare contracted nursing homes, hospitals and other care sites as needed. · Conduct home visits as needed for medical interventions. · Participate in the development and implementation of participant plans of care as part of and in collaboration with the interdisciplinary team. · Function as a member of the interdisciplinary team. Maintains regular attendance at, and participate in interdisciplinary team meetings; communicate participant changes, collaborate on care planning decisions and coordination of 24 hour care delivery. · Integrate the primary care treatment plan into the overall plan of care developed in collaboration with IDT (Interdisciplinary Team) · Interact with team members to meet emergent and acute needs of participants.. · Reviews all hospital, ED or other acute settings admission information and assists the IDT team with the discharge plan and services needed to ensure a safe transition from one level of care to another. · Provide preventive health care and maintenance for participants, including immunizations, screenings and monitoring of pertinent indicators. Follow the Eldercare’s clinical protocols · Prudently prescribe medications, therapies and other treatments for participants. · Demonstrates advance knowledge of the general diagnostic and therapeutic procedures for the at risk participants of the Eldercare PACE program · Interviews patients and completes a full comprehensive medical history and physical examination within PACE regulations and Eldercare policies. Qualifications: · State of Florida Advanced Practice Registered Nurse License; ACLS Certification · Advanced Practice Registered Nurse, Geriatric Certification preferred. Assessment of chief compliant and notification of physician within 15 minutes of arrival to procedure area. · Obtains and documents an accurate systems assessment at the beginning of each procedure. · Responds to diagnostic data and initiates appropriate action or intervention. · Assesses patient education needs on admission, and psychosocial needs specific to patients age, growth, and development. · Initiates plan of care within eight hours of admission. Establishes priorities for patient care based on patient needs as evidence by completion of assigned duties within shift. · Performs Pre-Operative and Post-Operative care and instruction for each assigned surgical patient during shift. · Performs dressing changes using aseptic technique. · Collects and labels specimens per policy when ordered by physician. · Performs bedside glucose monitoring according to policy, performs ostomy care to include irrigation, appliance usage and reinforcement of teaching. · Inserts intermittent or indwelling urinary catheter using aseptic technique. · Administers oxygen therapy according to physician order. · Transfer patients in/out of the GI station in an efficient manner, by obtaining a complete and correct set of physician orders. Completes documentation of transfer summary. · All other duties as assigned.
