Professional Case Management Certification preferred. (CCM or ACM) Work Experience : Minimum three to five years’ experience in Acute Care setting. Medical/Surgical and/or ICU experience preferred. Case Management experience preferred. Home Health/Hospice experience preferred. Knowledge, Skills, and Abilities : Excellent interpersonal communication and negotiation skills. Able to communicate effectively and work with people of all social, economic, and cultural background. Self-motivated, proven written, telephonic, and electronic communication skills, assertive and persuasive in interactions with customers, peers, management and core staff served. Qualifications & Requirements Education Graduate from an accredited school of nursing. Experience Prefer a minimum of one (1) year clinic experience Licenses/Certifications Have and maintain current licensure as a Licensed Practical/Licensed Vocational Nurse within the state of employment.
