Catastrophic Nurse Case Manager - Chicago, IL and northern Indiana

We are seeking an experienced Nurse Case Manager interested in working with injured workers and worker’s compensation cases to cover the Chicago, IL, greater area, including northern Indiana. This role is a home-based position with travel, up to 2 hours one-way (4 hours round trip), to doctors’ offices, hospitals, and various other locations. This individual is responsible for the medical case management of work-related injuries, which includes assessment, planning, coordination, implementation, and evaluation of injured/disabled individuals. The Case Manager works with insurance carriers, medical care providers, attorneys, employers, and employees, and closely monitors the progress of the injured worker and reports results back to the employer and insurance carriers. Qualifications Current, unencumbered registered nurse (RN) license in IL Willingness to obtain IN RN license Experience in worker’s compensation catastrophic case management is preferred. Experience may include: Critical care experience, catastrophic injury/illness case management, and/or disability management Clinical experience in acquired brain injury, spinal cord injury, multiple trauma (including amputation), and/or burns Recommended certifications (CCM, CRRN, CDMS, CRC). REQUIRED within two years of the date of hire Ability to occasionally work outside of traditional business hours (evenings or weekends) if needed to meet the needs of the claim Skills needed: organization and timeline adherence; PC and technology; communication (written, verbal and interpersonal) Valid driver’s license, and good driving history

Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient. Prepares clinical notes and updates the case manager after each visit as appropriate. Communicates with the physician regarding the patient’s needs and reports any changes in the patient’s condition; obtains/receives physician’s orders as required. Communicates with clinical supervisor and/or weekend coordinator as needed to assure continuity of care. Responsible to come into agency as needed to receive assignment, supplies. Communicates with clinical supervisor and/or weekend coordinator regarding new referrals and hospital discharges. Accountable to have all on-call equipment and supplies to appropriately carry out responsibilities. Understands role of Supervisor on Call / Administrator on Call and Weekend Coordinator to utilize for resource and emergency back up after hours. Completes all required documentation, adheres to electronic and paper documentation submission time frame. Represent the agency as a 24-hour service, respond to phone calls within agency time-frames, and make visits in accordance with agency policy. RNs supervises LPNs and Home Health Aides. Commits to 100% patient and customer satisfaction by always exhibiting a courteous and helpful manner during interactions with others, including patients, families, visitors, physicians, students and co-workers. Required Qualifications Current state of Illinois nursing license RN as applicable. Current CPR is required by AHA. One to two years of recent acute care experience in an institutional setting required. One year of home health care experience preferred. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist. Possesses and maintains a valid driver’s license and current automobile insurance and has the availability of personal and dependable mode(s) of transportation to conduct home visits. JOB SUMMARY Responsible for performing the daily activity calendar requirements set by the Activity Director. Required to support and follow the Activity Program’s mission, philosophy, policies and procedures and state and federal regulations.
