NP ROLE AND RESPONSIBILITIES: Provide primary and preventative care for long term care patients residing in skilled nursing facilities. Manage Residents with acute change in condition. Advanced care planning and chronic care management. Prescribe appropriate medications, including controlled substances in accordance with KY Law. Order, review and/or follow-up with treatments and diagnostic testing. Communicate with the nursing home staff on a regular and as needed to discuss changes in condition. and/or the care plan. Collaborate with primary care physician. NP QUALIFICATIONS AND EDUCATION REQUIREMENTS Current, unrestricted NP license in the state of Kentucky. Board Certification. DEA licensure. Clinical background in adult, family or geriatric specialties. Experience working in a long-term care setting. Ability to work with an EMR. Ability to manage patients with advanced illness and multiple chronic conditions. Ability to work within an interdisciplinary care team to coordinate patient care. Description Attention all Nurse Practitioners, we are calling you to partner with us as independent contractors providing services to our Military Service members. We primarily work weekends, with some occasional weekdays under the Reserve Health Readiness Program (RHRP) in the State of Kentucky on an "as needed basis". We provide health readiness services to meet the medical and dental requirements to maintain a deployable military force for the following: U.S. Army Reserve (USAR) Army National Guard (ARNG) U.S. Navy Reserve (USNR) U.S. Marine Forces Reserve (MARFORRES) U.S. Coast Guard Reserve (USCGR) Air National Guard (ANG) U.S. Air Force Reserve (USAFR) Responsibilities: Support Physical Health Assessments of military personnel. Review health history, service member medical records, and medical questionnaires. Interview military personnel to determine current and prior medical concerns. Identify any concerns that need to be addressed immediately, inclusive of Behavioral Health. Document interview results for readiness classification in the service member’s medical record using the proprietary system. How you make a difference The Nurse Practitioner (NP) under the supervision of the Site Medical Director and other site Physicians, offers a comprehensive range of medical services to incarcerated patients in accordance with established policy and state guidelines. They ensure necessary documentation of services is provided to the Site Medical Director or designee to monitor the provision of clinical services. Additionally, they communicate scheduling changes to the Medical Director and H.S.A. and make arrangements for coverage of medical services if unavailable for an extended period of time. Key Responsibilities Provides medical care to inmates, including conducting exams, chronic care clinics, and infirmary rounds. Maintains accurate and legible progress note documentation in SOAP format and adheres to formulary guidelines. Responds to emergencies and makes logical decisions regarding non-formulary drugs or off-site referrals. Supports existing policies and procedures for admission, transfer, and specialty consultant referrals. Maintains confidentiality and provides input into facility committees as requested. Additional Details -This is a full-time/day shift position. -This position offers an $8500 Bonus for full-time employment. Qualifications & Requirements Education A graduate of an accredited school of nursing An advanced degree in an approved and accredited Nurse Practitioner program. Experience Prior medical/surgical and/or emergency/trauma experience and corrections experience is preferred. Must have the patience and tact to deal with the inmate patient population and ability to work effectively in the corrections environment. Scheduling flexibility is also required to be able to rotate hours and shifts, if needed, and to be called during emergency situations to provide coverage. Licenses/Certifications Must have and maintain current licensure as a Nurse Practitioner within the state of employment. Must have and maintain current CPR certification.
