Position: Advanced Practice Provider (NP/PA) Monogram Health is seeking compassionate and skilled Advanced Practice Providers (Nurse Practitioners or Physician Assistants) to deliver high‑quality in‑home care to patients living with multiple chronic conditions, including diabetes, hypertension, chronic kidney disease, heart failure, COPD, depression, and other complex health needs. This role works within an integrated multidisciplinary care team (Clinical, Behavioral Health, Pharmacy, Administrative), establishing long-term patient relationships and providing continuity of care to complex patients in their homes and via telehealth. Highlights Schedule The Float APP will be required to provide in-home care to patients in the following locations: Austin, New Braunfels, and San Antonio. This role offers a clustered schedule, meaning you’ll see patients in one area per day rather than traveling long distances between cities. Compensation The base salary range for this role is $122,000 to $158,600 and reflects the range of experience and qualifications we may consider. Final compensation will be determined based on geographic location and job-related factors such as experience, skills, education, certifications, and internal compensation considerations. While a larger range is posted, most offers will typically fall within a narrower band based on these factors. In addition to base salary, this role is eligible for additional compensation, including bonus opportunities and a comprehensive benefits package. Responsibilities Provide evidence‑based, patient‑focused care that reduces avoidable hospitalizations and improves quality of life directly in the patient’s home Perform comprehensive assessments, including initial evaluations, annual wellness exams, follow‑ups, acute urgent visits, and post‑discharge visits. Educate patients and caregivers on disease management, preventive care, medications, and diagnostic testing Participate in recurring virtual integrated care team meetings Engage in quality assurance activities, including audits and documentation reviews, to ensure compliance with state board requirements and clinical best practices May practice in a variety of care settings beyond the patient’s home, including skilled nursing facilities (SNFs), assisted living facilities (ALFs), hospitals, and other affiliated clinical settings, as applicable. Position Summary The Pain Management Advanced Practice Provider (NP/PA) supports high-quality, efficient care for patients with acute and chronic musculoskeletal, spine, and neurological pain conditions. Working collaboratively with pain management physicians, orthopedic and spine specialists, and the clinical care team, the APP performs patient evaluations, coordinates treatment plans, manages follow-up care, and supports interventional and noninterventional pain management services. This role plays a critical part in improving patient access, continuity of care, functional outcomes, and overall patient experience. This is a part-time position (24 hours per week) in the outpatient setting. Key Responsibilities Clinical Assessment & Patient Management Perform comprehensive pain focused evaluations, including detailed histories, physical exams, and functional assessments. Evaluate and manage patients with spine related pain, joint pain, neuropathic pain, postsurgical pain, and chronic pain conditions. Develop and document individualized, evidence-based pain treatment plans, including interventional, pharmacologic, and conservative therapies. Order and interpret diagnostic studies such as imaging, labs, and electrodiagnostic tests relevant to pain management. Monitor treatment effectiveness and adjust care plans accordingly while identifying red flag symptoms requiring escalation. Interventional & Procedural Support Assist with or independently perform (as credentialed) pain management procedures such as injections and related interventions. Conduct preprocedural evaluations, consent discussions, and post procedure follow-up visits. Monitor patients for complications and address post procedure concerns promptly. Provide patient education regarding procedures, expectations, risks, and recovery. Care Coordination & Continuity Coordinate referrals to physical therapy, behavioral health, imaging, surgical services, or specialty providers as appropriate. Collaborate closely with orthopedic surgeons, spine specialists, and referring providers to align treatment plans. Ensure timely follow-up on diagnostics, procedures, and clinical recommendations. Support transitions of care for preoperative, postoperative, and long-term pain patients. Documentation, Compliance & Quality Maintain accurate, timely, and compliant clinical documentation. Support quality improvement initiatives related to pain outcomes, patient safety, and access to care. Adhere to regulatory standards, prescribing guidelines, and pain management best practices. Uphold all privacy, safety, ethics, and compliance standards. Team Collaboration & Education Work collaboratively with physicians, nursing staff, therapists, and administrative teams. Participate in care planning discussions, clinical huddles, and interdisciplinary collaboration. Educate patients and staff on pain management strategies, treatment options, and expectations. Qualifications Graduate of an accredited Nurse Practitioner or Physician Assistant program. Current Texas licensure as an NP or PA. National certification (AANP/ANCC for NP or NCCPA for PA). BLS required; ACLS and additional certifications preferred. Minimum of 2 years of clinical experience preferred; pain management, orthopedic, or spine experience highly valued. Strong clinical judgment, communication, and documentation skills. Ability to work effectively in a fast paced outpatient specialty practice. Physical & Work Environment Requirements Ability to sit, stand, and walk for extended clinic hours. Occasional lifting with assistance as required for patient care activities. Exposure to clinical environments including procedures, imaging, and patient care settings. Ability to work extended or adjusted hours based on clinic schedules and patient needs.
