JOB SUMMARY The Registered Nurse (RN) Charge is a licensed professional who uses the BSWH nursing professional practice model to coordinate patient care delivery by the health care team. In the capacity of Charge Nurse, works in the best interests of BSWH and accepts increased responsibility to support and carry out BSWHs mission and goals. Using the nursing process, the RN assesses the patient, identifies nursing diagnoses based on responses to health problems, develops and implements an individualized plan of care, and evaluates the patient's response. Reviews, revises, and updates medical records within facility guidelines and timeframes. Transcribes and implements physician's orders according to facility policy; documents care, progress, response to intervention in medical record in an accurate, thorough and concise manner. Follows up on client care responsibilities, communicates incomplete assignments, and current client needs to staff to assure continuity of care. Implements staff assignments and delegation of duties using appropriate judgment and knowledge; Demonstrates positive willingness to assist staff and patients; functions as an advocate for patient needs/safety Utilizes nursing knowledge to successfully meet the needs of the facility, patient population, and staff members utilizes nursing resources to enhance evidence-based practice Float among various clinical services, where qualified and competent and performs other duties and projects as assigned.

These services are performed in accordance with the physician’s orders and the established plan of care, under the direction and supervision of the Branch Director. Qualifications 1. Must hold a current and unencumbered Registered Nursing License in the state which you intend to work.2. Must be organized, detail-oriented and possess effective communication skills both orally and in writing. 3. RN must understand the issues related to the delivery of home health care and be able to problem-solve effectively. 4. Ability to assume a flexible work schedule. Requirements :1.
