Responsibilities Our Registered Nurses (RN) are responsible for the delivery and supervision of patient care through the nursing processes of assessment, diagnosis, planning, implementation, and evaluation. Essential Duties and Responsibilities Provide direct patient care to infants, children, and adolescents primarily in their home according to the active plan of care, as given by the physician and other appropriate personnel. Provide services in accordance with a physician’s orders and in adherence to state, federal, and national accreditation commission regulations; under the direction and supervision of clinical supervision staff and agency administrator. Perform comprehensive patient assessments. Administer prescribed medications, treatments, and therapies as ordered by the physician. Document timely clear, concise, and accurate clinical notes in accordance with the plan of care. Notify the RN Case Manager of any changes in the patient’s condition. Provide and document education to patients and their families on specific details of diagnosis, treatment, and prevention. Collaborate with other professional disciplines to ensure that patients are receiving quality care, necessary to achieve, and exceed individual goals. Use appropriate infection control procedures and safety measures. Comply with all company policies, practices, and procedures. Other duties as assigned. Qualifications Education and Work Experience Requirements Current State Registered Nursing License or compact license where applicable. One (1) Year Home Health Experience. One (1) Year Pediatric Experience. Current CPR Certification. Reliable Transportation. Other Qualifications and Skills Must have reliable transportation to office and patients' homes. Physical Demands/Working Environment Position regularly requires bending, reaching, standing, stooping, sitting, twisting, talking, and hearing. JOB SUMMARY Provides professional nursing care within the Intensive Care Unit; coordinates care planning with other disciplines. Provides care the meets the psychosocial, physical and general aspects of care. Determines the priorities of care based on physical and psychosocial needs of patients, as well as factors influencing patient flow through the system. Communicates with physicians regarding changes in patient’s status, symptomatology and results of diagnostic studies. Responds quickly and accurately to changes in condition or response to treatment. SUMMARY OF ESSENTIAL JOB FUNCTIONS Provides direct care to the ICU patients. Ability to perform a head-to-toe assessment on all patients and reassessments as per policy. This includes pediatric, adolescent and geriatric patients and the general patient population. Ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management. Ability to monitor hemodynamic status of patient and correctly interpret results. Demonstrates knowledge of cardiac monitoring; identifies dysrhythmias and treats appropriately. Ability to perform waived testing (point-of-care testing, glucometers) per Clinical Laboratory’s and the patient care unit’s policies and procedures. Ability to interpret the results of waived tests (point-care-care testing, glucometers); takes appropriate action on waived test results. Maintains current knowledge of medications and their correct administration based on age of the patient and his/her clinical condition. Follows the six (6) medication rights and reduces the potential for medication errors. Formulates a teaching plan based upon identified learning needs and evaluates effectiveness of learning, family is included in teaching appropriate care. Demonstrates the ability to assist physicians with procedures and performs services requiring technical and manual skills. Interacts professionally with patient and family and involves patient and family in the formation of the plan of care. Ensures an adequate stock of supplies and proper functioning of equipment. Responds promptly to changes in the workload, adjusting assignments as necessary. Performs other related duties as assigned or requested. Provides resource information to other staff. Schedule PRN Days PRN Nights Department: Women's Center Description: Looking for an individual that provides direct patient care in the Labor and Delivery setting. Provides assessment and planning for individualized patient care. Communicates with physicians about changes in a patient’s clinical condition including fetal monitoring and results of diagnostic studies. Responds quickly and accurately to changes in condition or response to treatment. Performs general nursing duties in all Maternal Child Health departments with adequate supervision. Able to assess neonate at delivery and perform neonatal resuscitation according to AHA/AAP guidelines. Knowledge of medications and their correct administration based on age of the patient and their clinical condition. Follows the 6 medication rights and reduces the potential for medication errors. Ability to formulate an individualized plan of care, revise plan as indicated by the patient’s response to treatment and evaluate overall plan daily for effectiveness. Demonstrates knowledge of fetal monitoring. Identifies FHR patterns and treats appropriately. Ability to perform a head-to-toe assessment on infants and adult females and reassessments as per policy. Ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management. Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors. Communicates appropriately and clearly to charge nurses, coworkers and physicians. Consults other departments as appropriate to provide for an interdisciplinary approach to the patient’s needs.
