Provides recommendations, patient/caregiver education/instructions and hospice support telephonically based on the situation and the plan of care. Collaborates with pharmacies, DME vendors and other agencies for effective patient management. Facilitates delivery or maintenance of provided medical equipment to meet patient needs. Assigns all visits, admissions and follow-up calls to on-call field staff (RN, LPN, HA, CH, SW) as needed. Submits accurate and detailed documentation in real-time to promote continuity of care. Utilizes a combination of agency resources and nationally recognized standards of practice to achieve excellent pain and symptom management and high-quality end-of-life care. Participates in agency performance improvement initiatives. Performs other duties as assigned. Qualifications Current, unencumbered license to practice as a registered nurse in the state you are assigned to work. One year of experience as a registered nurse. Current CPR certification. Knowledge of physical, psychosocial, and spiritual needs of terminally ill patients and their caregivers. Must be comfortable with technology. Must be willing and eligible to obtain additional RN licenses in other states (reimbursed). Preferred Previous hospice experience. Telephone triage experience. Spanish speaking. Director of Nursing (DON) A nursing home is seeking to hire an experienced full time Director of Nursing. Director of Nursing (DON) Duties Include: (but are not limited to) Access analyze and develop systems within the nursing department. Responsible for oversight of the daily clinical and administrative operations of the nursing department to assure that each resident receives the necessary care and services to attain or maintain the highest practicable physical, mental, and psychosocial wellbeing in accordance with their comprehensive plan of care. Assure that the nursing department’s philosophy and objectives incorporate evidence-based practice in providing high quality care and quality of life to each resident. Serve as an advocate for the residents and staff under his/her direction. Foster high standards of resident care through the provision and promotion of diligent orientation and continuing education efforts, adequate levels of nursing personnel, properly prepared nursing supervisors and managers, and current policies and procedures. The ability to assess, analyze and develop systems within the nursing department. Must have previous experience as a Director in Long Term Care and possess strong supervision and leadership skills. Reliable means of transportation Current CPR certification Strong communication and interpersonal skills Essential Functions/Areas of Accountability Essential Functions/Areas of Accountability Initiate actions independently and responsibly to improve the quality of client care Perform client visits as assigned to provide skilled interventions with a focus for achieving realistic client outcomes within a specified time period Provide direct care and assign and/or delegate functions according to education and demonstrated competence Act as client advocate Assess and evaluate health status of assigned clients: Collect objective and subjective client information Analyze, report and record data Validate, refine and modify data Utilize all data to identify and document current health care needs of assigned clients Collaborate with client, family/others, and other members of the healthcare team to identify client needs, and establish realistic goals of care Review plan of care on a regular basis to evaluates client response to plan, prioritize needs and modify plan to meet client’s current needs: Cultural, ethical, spiritual aspects and decisions regarding treatment Plan interventions to support human functions and maintain hygiene in a comfortable and safe environment Align with community resources as indicated for appropriate care Verify medical orders are accurate, properly authorized and without contraindication. Administer prescribed medications and IV therapy or provide client/family/other education regarding self-administration. Develop, initiate, and document plan to provide client/family/other education and counseling. Evaluate home environment and initiate plan to ensure a safe environment. Institute standard policies and procedures to stabilize clients in an emergency situation.
