What you'll do The Registered Professional Nurse III, Ambulatory Care is accountable for the delivery of safe patient care utilizing the nursing process. The RN assesses, plans, delivers, coordinates, integrates, and evaluates nursing care related to patient needs. The RN contributes to the support of patients through interdisciplinary teamwork. The RN III provides leadership to department staff, serving as a resource and mentor. These RNs contribute to the body of professional nursing practice through participation in research, committee work, and professional organizations, internal and external. Provides nursing care and services to patients throughout the life span and in various settings consistent with educational preparation. Collects and records relevant patient health data accurately and works in collaboration with the care team to contribute to the plan of care. Analyzes assessment data to identify patient problems and utilizes date to continuously meet care coordination needs. Identifies expected outcomes individualized to the patient. Evaluates patient response to plan of care and works in collaboration with the inter-professional team to make any needed adjustments. Implements and records interventions appropriate to diagnosis or patient problem. Evaluates the patient’s progress toward attainment of identified goals and initiates or participates in patient outcome improvement activities. Adheres to approved, research-based practice standards. Directs and supervises patient care provided by LPNs and support personnel. Performs and documents Point of Care (POC) testing and quality control in accordance with established laboratory policies and procedures. Maintains and documents testing reagents/supplies and equipment according to established laboratory procedures. Acts effectively as a group leader or preceptor when assigned. Advocates for patients to ensure that care preserves and protects the patient’s autonomy, dignity and rights. Delivers care in a non-judgmental and non-discriminatory manner that is sensitive to patient diversity. Assures that patients have informed consent signed for indicated procedures, 100% of the time, as measured by random supervisory chart review. Acquires and maintains current knowledge and skills in his or her area of practice. Provides proof of current BLS validation to supervisor. Completes required unit credentialing according to departmental policy by providing documented evidence of completion to supervisor. Provides to supervisor /manager evidence of completion of all required training and competency validation. Develops a plan of care that describes interventions to attain expected outcomes, with no more than 2-3 exceptions, as measured by random supervisory chart review and/or direct supervisory observation. Implements and records interventions appropriate to diagnosis or patient problem, with no more than 2-3 exceptions, as measured by random supervisory chart review and/or direct supervisory observation. Evaluates the patient’s progress toward attainment of identified goals, with no more than 2-3 exceptions, as measured by random supervisory observation and random supervisory chart review. Adheres to approved, research-based practice standards, with no more than 2-3 exceptions, as measured by direct supervisory observation and random supervisory chart review. Directs and supervises patient care provided by LPNs and support personnel 100% of the time according to department guidelines, as measured by co-worker feedback and managerial observation. Performs and documents Point of Care (POC) testing and quality control in accordance with established laboratory policies and procedures, with no more than 1 exception per year, as observed by supervisor and POC staff. Maintains and documents testing reagents/supplies and equipment according to established laboratory procedures, with no more than 1 exception per year, as observed by supervisor and POC staff. Acts effectively as a group leader or preceptor when assigned, as determined by co-worker or preceptee feedback and direct managerial observation. Advocates for patients to ensure that care preserves and protects the patient’s autonomy, dignity and rights. Delivers care in a non-judgmental and non-discriminatory manner that is sensitive to patient diversity, 100% of the time, as measured by no significant, substantiated patient or peer complaints received by the manager/supervisor. Assures that patients have informed consent signed for indicated procedures, 100% of the time, as measured by random supervisory chart review. Acquires and maintains current knowledge and skills in his or her area of practice. Provides proof of current BLS validation to supervisor. (To achieve excellence, one must do this in a proactive, timely manner prior to due date) Completes required unit credentialing according to departmental policy by providing documented evidence of completion to supervisor. Provides to supervisor evidence of completion of all required training and competency validation. (To achieve excellence, one must take personal responsibility for this task and complete by due date) Performs Similar or related duties as requested or directed. Triennial completion of professional nursing pathway for current practice level and provides evidence of completion to supervisor. Provides an annual in-service for co-workers and provides documented evidence of activity to supervisor. Demonstrates clinical problem-solving ability through written exemplar, patient/family feedback, co-worker report or direct managerial observation. What we need from you Education: 2 year / Associate Degree in Nursing, required or... Vocational / Technical Diploma Professional Nursing Program, required or... 4 year / Bachelor's Degree in Nursing, required All RN’s hired and do not currently hold a BSN will be expected to demonstrate enrollment in a bachelor’s prepared nursing program within 4 years of hire. BSN must be completed within 10 years. RN’s licensed on or before 12/19/17 are/remain exempt from this requirement. What you'll do The Certified Nursing Assistant (CNA) performs various resident care activities and services necessary in caring for the personal needs and comfort of residents. Nurse aides provide assigned residents with routine daily nursing care in accordance with our established nursing care procedures. Observes and reports resident’s status changes and needs. Answers call lights and alarms and attends to/reports resident’s needs Obtains weights, vital signs and reports results. Prioritizes and organizes work within specified time frames. Completes Intake/Output monitoring as appropriate. Obtains specimens and prepares them to be transported to the lab as needed. Provides/assists residents with ADL’s, including bathing, skin care, toileting, grooming, and dressing while encouraging the resident to participate at highest practicable level, and documents care provided accurately and timely. Assists/ feeds residents with meals and supplements. Assists clients with elimination needs, and/or provides incontinent care with each occurrence. Transfers, moves and repositions clients every 2-3 hours and prn. Maintains CNA II Requirement (if appropriate). Assists residents to move to activities area or dining room area, etc. Keeps unit neat, relaxed and homelike. Assists with admissions, transfers and discharges. Assists/provides residents with PROM and AROM, and ambulation goals as appropriate. Provides treatments as ordered on assigned residents. Operates all equipment and performs all procedures/care in a safe manner. Demonstrates proper body mechanics in all functions. Cleans and stocks supplies, and equipment as directed. Receives and directs phone calls promptly and accurately following Hospital etiquette guidelines. Demonstrates ability to accept constructive criticism and uses it to improve work performance. Demonstrates the skills and judgment necessary to provide non–direct patient care/support services to staff under the direction of licensed personnel. Maintains NYS CNA requirements. Promotes effective working relations and works effectively as part of a department/unit team inter and intra departmentally to facilitate the department's/unit's ability to meet its goals and objectives. Assumes all other duties and responsibilities as necessary. Completes work assignments on time/readily accepts assignments as observed by Director.
