Job Summary: Evergreen Healthcare Group is seeking a Travel MDS Coordinator (RN) to support skilled nursing facilities primarily throughout Wyoming , with occasional coverage in other states as needed. This role is responsible for coordinating and completing the Resident Assessment Instrument (RAI) process, including MDS, Care Area Assessments (CAAs), and Care Plans, while ensuring compliance with state and federal regulations. The Travel MDS Coordinator partners with facility teams to support accurate reimbursement, regulatory compliance, and high-quality resident care across multiple locations. Essential Functions: Occasional t ravel to assigned skilled nursing facilities to provide MDS and reimbursement support. Coordinate the interdisciplinary team (IDT) in the timely completion of resident assessments. Utilize MDS scheduling tools to establish and manage Assessment Reference Dates (ARDs). Communicate ARD selections and assessment timelines to facility IDT members. Complete Care Area Assessments (CAAs) as required. Assist with development and review of resident care plans related to MDS assessments. Monitor medical record documentation to ensure it supports accurate MDS coding and reimbursement. Conduct MDS audits and provide education, training, and mentoring to facility staff. Support Medicare and Managed Care utilization management processes as assigned. Assist facilities with regulatory compliance and reimbursement optimization. Collaborate with regional and facility leadership to identify opportunities for process improvement and clinical excellence. Education, Licensure, and Experience: Required: Current Registered Nurse (RN) license in good standing. Ability to obtain and maintain licensure in assigned states as required. Minimum of two years of MDS Coordinator experience in a skilled nursing facility (SNF) setting. Strong knowledge of RAI, MDS 3.0, Medicare PPS, PDPM, and regulatory requirements. Computer proficiency and experience working within electronic medical record systems. Willingness and ability to travel occasionally , including possible overnight travel. Preferred: MDS Certification. Multi-facility or consulting experience. PointClickCare (PCC) experience. Experience training and mentoring clinical teams.
