Position Summary The Licensed Practical Nurse delivers nursing care to assigned patients in collaboration with the Staff Registered Nurse/Advanced Registered Nurse, and is accountable for patient care administered. Education College Diploma in Nursing - Required Experience 1 - 2 Years - Preferred Two years Rehab and/or Medical/Surgical in the last five years preferred. License & Certification Professional License with State of Practice - Required Basic Life Support (BLS) - Required Core Job Functions Documents all patient related clinical information and assesses patient for special needs and plans appropriate intervention. Comprehensive review of the patient's medical history is performed and documented. Engages resources to facilitate timely discharge and throughput starting on admission. Anticipates forecasted patient care discharge needs to ensure throughput targets met. Provides input that decreases inefficiencies for unit, staff in collaboration with the manager. Contributes to formulation of nursing careplan for patients in consultation with the RN and in collaboration with patient, families, healthcare members using established nursing diagnoses and nursing protocols. Prioritize nursing care needs of clients. Assist in the review and revision of nursing care plans with the RN to meet the changing needs of the patient. Complies with regulatory guidelines for work area and professional practice standards. Assists with data collection, trouble shooting, and educating staff of performance improvement indicators. Utilizes available safety equipment and processes for patient care. Care plan development and implementation with health care team members collaborating to develop long and short term patient goals. Communication with physicians using Situation, Background, Assessment, and Recommendation format. Consistent rounding and communicating including present on admission indicators and baseline FIM scores. Contributes to the multidiscipline team conference. Administers medication utilizing the 5 rights and available safety equipment and processes. Utilizes clinical judgment with patient assessment and prescribed therapies to ensure promotion of goal obtainment. As a leader holds healthcare providers accountable for the standard of care. Utilizes Teach Back technique to assess understanding of education. Assesses needs for discharge and the best way the patient may be provided instruction. Utilizes appropriate written communication, education packets and/or audio-video instructions. Teach back performed with patients to facilitate patient outcome improvements by increasing knowledge and skills needed to improve healthy behaviors. Elicit information using effective counseling techniques. Identify factors that might affect communication or learning.
Position Summary The Registered Nurse maintains standards that relate to patient care and technical services performed and for the pre, intra and post diagnostic and therapeutic cardiac procedures in the invasive cardiology labs by serving in the roles of monitoring, circulating and scrubbing procedures. The focus of these standards is to assure optimal examinations, interventions, and post care while maintaining the highest level of patient safety and comfort possible. Education Associate Degree in Nursing - Required Experience 1-2 Years Critical Care - Preferred 1 Year Cath Lab or EP Lab or Special Procedures - Preferred License & Certification Registered Nursing License with State of Practice - Required Basic Life Support (BLS) – Required Advanced Cardiovascular Life Support (ACLS) - Required within 90 days from hire date Core Job Functions Demonstrates knowledge and skill to provide care appropriate to any age-related needs of the patient being served. Demonstrates principles of growth and development appropriate to the patient populations served. Coordinates patient care needs with team members and other appropriate departments. Assesses and documents the physiological health of the patient. Participates in patient/family education and teaching. Ensures that all patients are prepped according to procedure guidelines. Maintains aseptic/sterile technique during invasive procedures per policy. Anticipates needs and provides procedural supplies and/or equipment to the physician. Proficient in arterial and venous access management. Accurately enters procedural documentation and patient charges per department guidelines. Major arrhythmias and hemodynamic events are identified and communicated to ensure proper action for patient safety. Maintains radiopharmaceutical logs and computer entries for period of time required by the DNR. Proficient knowledge in the management of cardiac and emergency medication. Accountable for standards of care as related to analgesia and/or moderate sedation. Compliant with the use of Pyxis for medication access per hospital policy. Ensures open communication between nurses and other team members. Promotes patient satisfaction in communication with patient and caregivers. Informs physician of patient assessment changes. Knowledgeable of Joint Commission, OSHA and CMA regulatory guidelines concerning the care of the invasive cardiac patient. Participates in quality improvement processes surrounding Surgical Care Improvement Project, AMI Core Measures, Door to Balloon time and American College of Radiology Standards to achieve highest level of performance. Assures team is included in patient safety activities such as Time Out, Read Back/Verify, Implant verification and procedural sponge and instrument counts.
