HOW YOU'LL WORK: You'll perform personal care including but not limited to: bathing, grooming, skin care, toileting, and oral hygiene. You will effectively communicate and document any changes in patient conditions to the registered nurse or supervisor and maintain positive relationships with your patient and team. You'll be responsible for all practices and duties within the home health aide scope of practice as outlined by the state for its HHA personnel. MAJOR AREAS OF RESPONSIBILITY: Patient Care: Assist patients as needed with lifting and transferring Communication: Maintain good relationships with the patient, the patient’s family, and staff. Reports any changes to the patient’s medical and living conditions to the supervisor or the case manager. Minimize Patient Risk: Ensure the living environment is healthy and safe. Policies: Completes all documentation following agency protocol and Medicare/Federal guidelines. Operations: Attend in-service training and mandatory agency meetings.
