Position Summary Blue Ridge Care is seeking a compassionate and experienced Nurse Practitioner to provide primary care services within our PACE program. Under the supervision of the Medical Director, this role performs comprehensive assessments, manages ongoing care, and partners closely with the Interdisciplinary Team (IDT) to support frail elderly participants. This position plays a vital role in developing care plans, treating acute and chronic conditions, and providing education to participants and caregivers while ensuring high-quality, coordinated care.
