Location Overview: University Crossing ***$2,500 sign on bonus*** Position Summary: Responsible for the nursing care planning and management of patients in collaboration with the multidisciplinary team. Responsibilities: Identifies patient care requirements by establishing personal rapport with potential and actual patients and other persons in a position to understand care requirements. Establishes a compassionate environment by providing emotional, psychological, and spiritual support to patients, friends, and families. Promotes patient's independence by establishing patient care goals; teaching patient, friends, and family to understand condition, medications, and self-care skills; answering questions. Assures quality of care by adhering to therapeutic standards; measuring health outcomes against patient care goals and standards; making or recommending necessary adjustments; following hospital and nursing division's philosophies and standards of care set by state board of nursing, state nurse practice act, and other governing agency regulations. Resolves patient problems and needs by utilizing multidisciplinary team strategies. Maintains safe and clean working environment by complying with procedures, rules, and regulations; calling for assistance from health care support personnel. Protects patients and employees by adhering to infection-control policies and protocols, medication administration and storage procedures, and controlled substance regulations. Qualifications: Registered Nurse licensed in the State of Florida. One year of skilled nursing or acute care experience preferred. Current hands-on CPR/BLS Certification. The Discharge Nurse prepares patients and caregivers for a safe transition from inpatient rehabilitation. The role coordinates discharge readiness, medication education, clinical follow-up needs, and communication across the interdisciplinary team to reduce avoidable complications and support a consistent discharge experience. Job Responsibilities: Assess discharge readiness 72 to 96 hours before planned discharge and identify unresolved education, caregiver-training, or clinical needs. Provide and coordinate education on medications, treatments, precautions, and condition-specific care needs. Engage specialty clinicians for wound, respiratory, infection-prevention, pharmacy, and other focused education. Review medications, instructions, warning signs, follow-up plans, and questions with patients and caregivers on the day of discharge. Escalate unresolved clinical or coordination concerns to the appropriate team member. Reconcile discharge medications, confirm patient understanding, and address pharmacy, insurance, prior-authorization, or formulary barriers. Review discharge instructions and follow-up needs, including appointments, precautions, equipment, home services, and ongoing treatment. Collaborate with physicians, case managers, bedside nurses, therapists, pharmacists, and community providers to resolve barriers. Participate in team huddles, conferences, and discharge planning rounds as assigned. Maintain accurate documentation and support monitoring of discharge-readiness, patient-experience, medication, and readmission outcomes. Participate in efforts to standardize and improve discharge and care-transition processes. Job Qualifications Requirements: Current Florida Registered Nurse (RN) license. Minimum three years of clinical nursing experience. Experience in inpatient rehabilitation, acute care, medical-surgical nursing, care coordination, or case management. Strong knowledge of medication management and patient education principles. Excellent communication, documentation, and collaboration skills. Preferred Certified Rehabilitation Registered Nurse (CRRN). Experience with discharge planning, care transitions, or population health. Experience working with interdisciplinary rehabilitation teams. Knowledge of Medicare, CMS, and IRF regulatory requirements. Provides for dietary and fluid needs of the patient such as nourishment, feeding, tube feeding, observing and recording IV fluids and maintaining an accurate fluid intake and output. Takes and record vital signs: temperature, pulse, respirations, and blood pressure. Qualifications Licensed as an LPN in the state of Florida. 1 year of experience in acute care or post-acute care inpatient hospital Required.
