Registered Nurse

Registered Nurse (RN) Responsibilities: Participate in the development and implementation of the service plan and initial assessments for clients receiving nursing services to ensure proper placement of staff Regularly assesses the needs of clients receiving Nursing services Assess and chart observation of the patient’s condition at each visit Dress or redress wounds and assess the healing process Provide education to patients and families on proper home health care procedures and strategies. Coordinate with Physicians, Physical Therapist, Occupational Therapists, and other individuals in the patient’s care plan Provide hands on care, (i.e., wound care, ventilator care etc.) Recommend tools or devices that may improve quality of life for the patients Regular evaluations and re-evaluations (as warranted by changes in condition but at least every 60 days) of the patient’s nursing needs Taking verbal orders Make referrals to other disciplines, as indicated by patient’s assessment Maintain a current and accurate patient medication profile Medication Administration Training Monitoring Medication Complete service plans Supervise CNAs and PCAs and Nurses Train new staff Supervise management of records Investigate incidents/grievances Investigate accidents Conduct orientation training Communicate timely and effectively with the agency on changes in visit assignment, dates of scheduled visits, and schedule changes to scheduler and patient care to ensure patient needs are met Recurring training requirements and training staff based on assessments and requirements.
