In this role, you will serve as the clinical face of our agency and report to our Director of Nursing. Your primary responsibilities will include conducting comprehensive medical assessments for incoming clients, performing mandatory routine supervisory visits to ensure high-quality care, and providing direct skilled nursing care for our PRN clinical cases managed through the Veterans Affairs (VA) system. Compensation & Pay Structure We offer premium, competitive pay scales tailored to the type of clinical work completed: Skilled Nursing Cases: Hourly pay rate up to $60 per hour (dependent upon case complexity and acuity). Assessments & Supervisory Visits: Flat rate up to $100 per visit for completing new client medical assessments and routine caregiver supervisory evaluations. Essential Responsibilities New Client Assessments: Conduct thorough in-home clinical assessments to evaluate new clients' medical, physical, and cognitive needs, establishing an accurate, compliant plan of care. Supervisory Visits: Perform routine, scheduled supervisory field visits for existing clients to review the care plan, assess client satisfaction, and personally observe and evaluate caregiver competency. VA Skilled Nursing Cases: Provide high-quality, PRN skilled nursing interventions for veterans enrolled in the VA community care program (including wound care, medication management, injections, or clinical monitoring as ordered). Clinical Documentation: Complete timely, precise, and compliant electronic charting and nursing notes in accordance with Georgia DCH regulations and VA guidelines. Care Coordination: Communicate effectively with agency leadership, families, and primary care physicians regarding changes in a client’s clinical status. Qualifications & Requirements License: Active, unencumbered Registered Nurse (RN) license in the State of Georgia. Experience: Minimum of 1–2 years of nursing experience; home health, community health, or VA clinical care experience is highly preferred. Skills: Strong clinical assessment skills, independent problem-solving, and a compassionate approach to elderly and veteran populations. Compliance: Must possess a valid Georgia Driver’s License, reliable personal transportation, and current auto insurance. Certifications: Current BLS/CPR certification.
