MDS Coordinator The Heritage Rehabilitation & Health Care Center is seeking an experienced MDS Coordinator to join our team. Responsibilities Coordinate and complete accurate, timely MDS assessments Ensure compliance with all federal, state, and CMS requirements Develop and update individualized resident care plans Monitor assessment schedules and submission deadlines Review clinical documentation for accuracy and reimbursement support Collaborate with nursing, therapy, social services, dietary, and other departments Track quality measures and assist with performance-improvement initiatives Educate staff on MDS documentation and regulatory requirements Qualifications Current New York State RN license Previous MDS experience in a skilled nursing facility Strong knowledge of MDS 3.0, PDPM, Medicare, and care planning Experience with Matrix preferred RAC-CT certification preferred Strong organizational, communication, and leadership skills Salary $100,000–$120,000 per year , based on experience.
