Job Responsibilities

• Receiving orders and care plans from a virtual care team to assist patients during their acute phase of illness
• Coordinating with virtual nurses and hospitalists to identify patient needs and any interventions they may require in the hospital at home setting
• Providing transition of care support and participating in home visits
• Obtaining information from patients about specific symptoms and medical questions
• Measuring and monitoring blood pressure, vital signs, and the overall condition of patients
• Providing timely and accurate reporting to the virtual care team
• Assisting patients with their activities of daily living (ADLs) such as personal hygiene, mobility, and toileting
• Offering a compassionate ear to patients and providing much-valued companionship
• Maintaining patient engagement by establishing rapport, demonstrating empathy, and building a trusting relationship

Qualifications and Skills

• Active, unencumbered New Jersey CNA license
• Experience working in an acute care, in-patient, hospital setting
• Current American Heart Association CPR for Healthcare Providers Certification
• Ability to lift a minimum of 50 lbs
• Hepatitis B and MMR records
• Current influenza vaccination
