General Information:

Job title: Long-term Care Licensed Practical Nurse (LPN)

Location: Crest View Nursing & Rehabilitation Center - New Lisbon, WI

Schedules Available:

  • Full-time, 75 hours per pay period; PM Shift, 2-10pm
  • Part-time 1, 52.5 hours per pay period; PM Shift, 2-10pm
  • Part-time 2, 22.5 hours per pay period; Day Shift, 6am-2pm
  • Part-time 2, 15 hours per pay period; Day Shift, 6am-2pm or PM shift, 2-10pm

Weekend rotation: Every other weekend

Holiday rotation: Every other holiday

Additional Information: **Up to $3,000 sign on bonus**

Position Summary:

The Nursing Home Licensed Practical Nurse position assesses and passes medication to residents, reports to physician any changes they have, reports to charge nurse, updates family members, and assists CNAs.

Position Responsibilities:

  • Administer medications to residents while following the 5 Rights
  • Report any allegations of abuse, neglect and misappropriation to Director of Nursing and Administrator
  • Report any change in condition to the charge nurse
  • Assist CNAs with answering call lights and other responsibilities
  • Update physicians, family members, etc. of change in condition as needed
  • Perform other duties as requested.

Position Requirements:

  • Must hold and maintain Wisconsin LPN license.
  • Current CPR/BLS certification required.
  • 1+ years of related work experience preferred.
  • Exceptional accuracy and attention to detail required.

Knowledge, Skills, & Abilities

  • Intermediate proficiency with computers is required.
  • Knowledgeable regarding medication administration.
  • Comprehensive nursing skills.
  • Strong quantitative and analytical competency.
  • Self-starter with excellent interpersonal communication and problem-solving skills.
