Pathways Nursing and Rehabilitation Center is seeking Licensed Practical Nurses to join our team! Pathways Nursing and Rehabilitation Center is a 112 bed complex care nursing home specializing in: all Pediatric needs, Adult Ventilators, Adult Traumatic Brain Injury and Rehabilitation. Recently underwent a complete building renovation, Pathways offers a pleasant and modern work environment. You will work alongside fellow caregivers to provide high quality patient care.

Responsibilities:

  • In collaboration with the RN collects data about residents and determines priorities is resident care based upon identified needs and goals.
  • Contributes to the plan of care through collaboration and effective communication with the RN and other members of the healthcare team.
  • Under the direction of the RN and within the scope of practice, complies with established nursing policies, procedures and standards to provide safe, quality care to residents.
  • Administers medications and monitors IV therapy as ordered and according to established policies and standards.

Job Types: Full-time, Part-time

Location : 2 Empire Drive, East Greenbush, NY Office Hours: Monday-Friday Days Key Responsibilities: Deliver direct and indirect patient care, adhering to individualized care plans in an outpatient environment. Ability to manage physical demands and maintain composure in a fast-paced environment. Pay Range:$22.85 - $28.95 Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
