Now Hiring – RN Case Manager – Liberty, MO Hours:M-F 8:30am-5pm The RN Case Manager will be responsible for assisting the Team with On Call/Case Management/Visits to aid in coverage and call outs. Job duties can include, but are not limited to: Completing an initial assessment of patient and family to determine hospice care needs Regularly re-evaluates the patients nursing needs Use assessment data to determine care plan needs Collaborate with the entire healthcare team, patient and family, to help direct the work of the team to provide are that is continuous and well-coordinated Counsels the patient and family in meeting end of life needs Initiates appropriate preventative and palliative nursing procedures Communicates with the physician regarding patient's needs and reports any changes in the patient's condition Supervises the Hospice Aides Education: Associate's Degree in Nursing Professional Experience: Must have a State Certified RN license 2 years' experience as a professional nurse, hospice experience preferred Must have a valid Driver's license and reliable transportation Must be willing to travel Strong verbal and written communication skills Ascend Hospice - is part of the Ascend family of Healthcare Companies. Overview Department: ICU Employment Type: Full Time Shifts Available: Nights Hours: 12-hour shifts - 7:00pm to 7:00am Location: Providence Medical Center - Kansas City, KS The Intensive Care RN (ICU RN) will be responsible for providing intensive care to patients with life-threatening conditions. The ICU RN will have advanced skills in critical care settings, make quick decisions, and will be compassionate to all patients. An ICU RN monitors vital signs, blood oxygen levels, administer IV Fluids, and communicates effectively to the medical team to modify care plans accordingly. Responsibilities Essential Duties and Responsibilities (includes, but not limited to): Deliver high-quality care to critically ill patients by performing detailed assessments, developing care plans, and implementing complex nursing interventions Use advanced equipment like cardiac monitors, pulse oximeters, blood pressure monitors, ventilators, infusion pumps, and capnography devices to continuously track vital signs and physiological parameters Safely administer medications, intravenous fluids, and blood products with precise dosing and timing, closely monitoring patient responses to ensure effectiveness and prevent adverse reactions Manage life-support devices such as ventilators, dialysis machines, and cardiac monitors Provide emotional support and education to patients' families, acting as a liaison to ensure they understand the care being provided Document all aspects of patient care to ensure continuity and compliance while rigorously maintaining infection control and safety protocols to protect vulnerable patients Qualifications Education, Work Experience & Qualifications: 1. Current and valid state RN License.2. Current BLS certificate upon hire and maintain current.3. Completion of Critical Care Course preferred. 4. Basic Arrhythmia Interpretation within 30 days of hire.5. Current ACLS certificate 30 days upon hire and maintain current. 6. Minimum of one year as a staff RN in acute care hospital, critical care preferred.7. Critical Care RN (CCRN) Certification preferred. 8.
