Job Description The Registered Nurse in the Sexual Health Center provides direct patient care, coordinates treatment plans, collaborates with interdisciplinary teams to ensure quality outcomes, maintains clinical documentation, adheres to safety protocols, and supports patient education and advocacy. The RN will work partly in the ambulatory fixed site at 200 South Street in Morristown, partly in the Morristown Medical Center Emergency Department, and partly in the community in an Atlantic Mobile Health vehicle and at community-based organizations through Northern New Jersey including, but not limited to, Morris, Sussex, and Passaic Counties. The Sexual Health Department in a small, interdisciplinary team that primarily promotes public health initiatives funded through grants and awards. Public health initiatives include prevention, testing, and treatment for HIV, HCV, and sexually transmitted infections; harm reduction health programming such as preventative care, acute and chronic disease management, and wound care; education and counseling to promote safer sex and use strategies. Principal Accountabilities Patient Assessment: Perform systematic, comprehensive, and accurate assessments. Assess potential discharge needs during the initial assessment process. Administers medications to patients and monitor effects of medications and treatments and report adverse effects in a timely manner. Assess patient for pain level. Provide appropriate intervention, reassessment, and documentation. Systematically monitor the maintenance of protocols, standards, and plans of care. Plans and provides individualized patient education. Clinical Skills: Perform nursing-related skills and activities competently. Analyze situations critically. Maintain complete and accurate documentation. Identify and document realistic long and short-term goals. Complete interdisciplinary plan of care record. Participate in interdisciplinary rounds. Evaluate the effectiveness of interventions and revise the plan of care as needed. Recognize and evaluate patient and family responses to therapies and nursing interventions. 3: Professional Skills: Communicate clearly and effectively with all members of the healthcare team. Review laboratory, pathology, and imaging results, communicating findings and provider recommendations to patients. Coordinate preoperative and postoperative care for surgical oncology patients. Identify urgent clinical concerns and facilitate timely evaluation and treatment. Patient Education Educate patients and caregivers regarding: Surgical procedures Preoperative preparation Postoperative recovery Wound care and drain management Medication management Nutrition and activity restrictions Signs and symptoms requiring immediate medical attention Reinforce physician treatment plans and ensure patient understanding. Provide emotional support and resources throughout the patient's cancer journey. Care Coordination Coordinate referrals to specialists, medical oncology, radiation oncology, genetics, nutrition, social work, and other support services. Schedule diagnostic testing and ensure timely completion. Ensure continuity of care across outpatient, inpatient, and procedural settings. Clinical Documentation Document nursing assessments, interventions, patient education, and communications accurately in the electronic medical record (EMR). Maintain accurate medication reconciliation and allergy documentation. Process medication refills according to practice protocols. Complete disability forms, FMLA paperwork, and other clinical documentation as appropriate. Telephone Triage Assess patient concerns using evidence-based triage guidelines. Provide clinical advice within scope of practice. Escalate urgent or emergent concerns to the provider. Document all patient communications in the EMR. Quality and Compliance Maintain compliance with HIPAA, OSHA, CMS, and organizational policies. Participate in quality improvement initiatives.
