The Grand at Barnwell is seeking LPNs for our Weekend Baylor Program!

Location: Valatie, NY (25-mins from Albany)
Salary Range: $33.75 – $44.50 per hour (Baylor rate applied)

Must successfully complete all shifts to earn Baylor rate - otherwise LPNs will earn the base rate.

Position Overview – LPN Weekend Baylor (Saturday & Sunday)

Join our dedicated nursing team as a Weekend Baylor LPN, working two 15-hour shifts Saturday & Sunday. Baylor LPNs earn 1.25x the base hourly rate ($27.00 to $34.00), with a pay range of $33.75 to $44.50 per hour, depending on years of experience (0–20+) and shift.

In this role, you will provide high-quality patient care by executing nursing processes, performing treatments, and supporting patients’ physical and emotional needs. You will collaborate closely with our CNA team to ensure timely, accurate care, manage medication administration, and maintain thorough documentation.

LPN Responsibilities

Patient Care & Service:
Promote and restore residents’ health by completing the nursing process; perform treatment procedures; provide physical, educational, and emotional support to residents, families, and visitors; supervise assigned team members.

Documentation:
Ensure concise, accurate, and complete resident care documentation using our computerized medical record system.

Support of CNA Team:
Direct CNAs in the timely completion of quality, individualized personal care, ensuring accurate and complete CNA documentation.

Medication Administration & Reporting:
Administer medications and provide/receive written and verbal reports as required by departmental protocol.

LPN Requirements

  • Current and valid NY LPN license

  • Experience in a nursing home or hospital is a plus

  • Valid CPR certification
