As a minimum, an unencumbered State of Illinois Licensed Practical Nurse, LPN license. Prior Licensed Practical Nurse, LPN experience is preferable but we are willing to invest in the right candidate!

As a Licensed Practical Nurse, LPN you are the backbone of the Clinical Department. Your work will ensure our residents receive the high standard of care they have grown to expect at Bella Terra Streamwood by ensuring residents receive the medication and nursing care they need as directed by their individual care plan, conducting resident assessments, being a team leader, and advocating for your residents. Communicate/report information to residents, resident families, physicians, guardians, governmental agencies, and facility personnel as required by facility policy and State/Federal guidelines. Ensures that all aspects of resident care plans are implemented and maintained. Make recommendations for care plan changes to the IDT per professional standards. Ensures compliance with infection control standards. Immediately corrects/addresses identified instances of non-compliance.
