Position Summary We are seeking a compassionate and experienced Admissions Registered Nurse (RN) to join our hospice team. The Admissions RN is responsible for evaluating patients for hospice eligibility, completing admission assessments, coordinating the start of care, and educating patients and families about hospice services. This role requires excellent clinical judgment, strong communication skills, and the ability to work collaboratively with interdisciplinary team members to ensure a smooth transition into hospice care. This position will oversee the Bergen/Essex/Union county area. The Hospice RN Team Lead is responsible for providing leadership and direction to the designated team of nurses to ensure the highest standards of patient care. The Team Lead is responsible for developing and implementing patient care plans, assessing patient needs, and ensuring that all care is delivered in accordance with established protocols and best practices. Responsibilities: Participates in the coordination and planning of the delivery of quality, cost-effective hospice care. Evaluates patient services provided by the interdisciplinary team. Maintains working knowledge of hospice state licensure, federal and accreditation requirements.
