Licensed Practical Nurse LPN - Position Summary:

Provides licensed nursing services to residents and may require supervision of other staff positions.

Licensed Practical Nurse LPN - Responsibilities:

  • Responsible for medication administration for the residents in compliance with Federal & State

  • regulations.

  • May be responsible for the admissions of new residents to include all admission assessments

  • and documentation.

  • Responsible for completing service plans.

  • May be responsible for clinical system tracking (i.e. wounds or infection control) under the

  • direction of the Director.

  • Provide general consultation regarding health‐related concerns within the community.

  • Ability to communicate effectively and efficiently when listening, speaking, reading and writing.

  • Notify physicians and/or family of any change in resident’s health and provides proper

  • documentation.

  • Respond quickly to all emergencies.

  • Work as part of a team and handle multiple tasks safely and effectively while maintaining close

  • communication with all departments and staff members.

  • Provide educational meetings for residents and staff and attend all required training, in‐service

  • and staff meetings.

  • May provide oversight and direction for care staff.

  • Understand roll in the Safety & Disaster Plan.

  • Maintain confidentiality of residents’ and prospective residents’ information.

  • Strive to maintain a safe working environment through the prevention of accidents, the

  • preservation of equipment and the achievement of safe working practices.

  • Maintain a positive and professional demeanor toward residents, visitors, families and co‐

  • workers.

  • Adhere to all policies and procedures of Morningside Elite Management.

  • Perform other duties as assigned.

Licensed Practical Nurse LPN - Requirements:

  • Must be a Licensed Practical Nurse LPN of the state. Diploma or Associates or Bachelor's to two years related experience and/or training; or equivalent combination of education and experience.

  • Must be Cardiac Pulmonary Resuscitation (CPR) certified or willing to become certified.

ESSENTIAL FUNCTIONS 1. In collaboration with Department Head, trains department staff, (new and current) to maintain skill levels and compliance with evolving mandates. 2. Lead LPN Supervisor for department staff on process, supply, and team-related needs, including tactfully diffusing difficult situations. 3. Role model for the department by way of promoting compliance with facility policies and procedures. 4. Reports irresolvable concerns to the department head/supervisor and is involved with disciplinary processes. 5. Administers medication and treatments according to the physician's orders. 6. Assumes responsibility for ordering medications. 7. Transcribes, documents, and implements physician's orders. 8. Collects data to assist in the nursing assessments during each shift. 9. Gives daily shift reports to CNAs assigned to the unit before the start of the shift. 10. Review written daily assignments for each resident on the assigned unit. 11. Assures assignments are completed before the end of the shift. 12. Gives and takes nursing reports on assigned units. Communicates essential information to appropriate parties. 13. Makes rounds on assigned residents at least once every two hours. 14. Collaborates with the Care Plan Coordinator to develop and maintain nursing care plans and update these plans regularly and as the resident's condition changes. 15. Attends and participates in interdisciplinary resident care conferences as requested. 16. Assists the attending physician with rounds and special procedures as needed. 17. Participates in the yearly written evaluations of nursing assistants and documents accurately their strengths, weaknesses, and the steps that need to be taken to improve their performance. 18. Assists Staff Development with orientation and on-the-job training for new nursing personnel as needed. 19. Collaborates with the nursing supervisor in documenting disciplinary concerns for nursing assistants on designated shifts, including reprimands, oral and written warnings, and corrective action recommendations. 20.
