Patriots Colony is hiring a 13-week Temporary LPN for Day Shift. Temporary positions require 2 years LPN experience.

Overview
Works under the supervision of a Registered Nurse, Nurse Manager or Nursing Supervisor, and ultimately responsible to the Director of Nursing or Assistant Director of Nursing and Director of Quality Care Management. The Licensed Practical Nurse (LPN) participates in the development, maintenance, or modification of nursing plan of care. Provides for the emotional, physical comfort and safety of the residents and their families. Assists in supervision of auxiliary personnel.

What you will do

  • Medications and treatments are administered accurately and according to provider's orders.
  • Allows resident choices within limits of capability considering safety /dignity issues and follows policies and procedures of facility to include resident privacy and dignity.
  • Assigns duties to non-professional staff such as CNAs according to education, training, policy and resident need in order to assure quality resident care is delivered. Engages team members for optimal performance and addresses performance as needed.
  • Contributes to the development, maintenance, or modifications to the individual service plans. Identifies changes in condition with proper documentation according to nursing practice standards. Demonstrates consistent follow through for positive resident outcomes. Utilizes good judgment and specialized skills.


Qualifications:
Experience

  • 2 years LPN Nursing experience (Required)

  • Licensed Practical Nurse (LPN) - Virginia Department of Health Professions (VDHP) Upon Hire(Required)

LPN - Hampton Primary Care

Hampton, Virginia Overview Performs duties under the direction or supervision of a licensed medical practitioner or a registered nurse. Responsible for conducting a focused assessment of the patient to include vital signs and gathering data, implementing established plans of care, and evaluating effectiveness of treatments in the medical practice setting. May float to other practices within the region providing operational support as needed. Newport News, Virginia Overview Responsibilities include work scheduling, evaluating employee performance and managing employee relations.
