Essential Job Functions/Responsibilites For Licensed Practical Nurse:

1.  Provides direct patient care as defined in the state nurse practice act.

2.  Implements plan of care initiated by the registered nurse. 

3.  Provides accruate and timely documentation consistent with the plan of care.

4.  Participates in coordination or home health services, appropriately reporting the identified needs of other disciplines to the registered nurse and/or clinical supervisor.

Qualifications:

1.  Home health experience preferred.

2.  Currently licensed as a Licensed Practical Nursing by the state. 

3.  Possess and maintain current CPR certification.

4.  Licensed driver with current automobile insurance.

5.  Demonstrates good verbal and written communication skills.

6.  At least 18 years of age.
