Hilltop Healthcare & Rehab is seeking a compassionate Licensed Practical Nurse (LPN) who is passionate about working with seniors and providing quality care.

Qualifications for Licensed Practical Nurse (LPN):

Active Licensed Practical Nurse (LPN) license

Duties and Responsibilities for LPNs:

Administer prescribed medications and treatments in accordance with nursing standards.
Monitor patients’ vital signs and report any changes in condition to the Registered Nurse (RN).
Assist in the development and implementation of patient care plans.
Provide direct care to residents, including assistance with daily living activities such as bathing, dressing, and feeding.
Perform routine diagnostic tests and assist with procedures.
Supervise Certified Nursing Assistants (CNA) and provide direction as needed.
Ensure that medical records are maintained and up to date, documenting all observations and interactions with residents.
Ensure a safe and comfortable environment for residents by maintaining cleanliness and organization of patient rooms and common areas.
Work collaboratively with the nursing team and other healthcare professionals to provide optimal care to residents.
Assist in the admission, transfer, and discharge process for residents.
