The Clinical Nurse Specialist (CNS) provides clinical leadership and expertise for population-based nursing care to integrate evidence based care, advance clinical practice to achieve optimal patient outcomes and facilitate transitions of care. The CNS provides leadership and mentoring to support the interprofessional team in the evaluation and implementation of evidence-based therapies and research. The CNS will collaborate with other work area leaders in the education, research, quality and clinical updates for unit staff to achieve the goals and objectives of the work area.

Provides clinical practice leadership through the design, implementation and evaluation of care delivery for patient population. Collaborates with the healthcare team to lead and manage projects impacting clinical care. Acts as a change leader to respond to regulatory, organizational and community needs in a dynamic healthcare environment.

Advances nursing practice for a defined patient population by developing policies, procedures, guidelines and education that incorporate evidence-based strategies related to nursing care delivery/response, documentation and communication. Partners with interdisciplinary team to address gaps and improve clinical knowledge and nursing practice.

Collaborates with physician leadership to co-lead clinical quality initiatives. Mentors nurses in the process of planning, implementing and evaluating change process. Cultivates a practice environment that promotes mutual respect, communication and collaboration.

MAJOR RESPONSIBILITIES/ESSENTIAL FUNCTIONS Estimated
The primary job duties this position is responsible for achieving are listed in order of importance.
  1. Collaborates with Nursing Director of MCH, NICU Manager and staff to assess learning and development needs of nursing staff including the evaluation of staff clinical abilities, and makes recommendations to Nursing Director.
  2. Develops individual teaching plans and provides direct education for specific patient population.
  3. Assists in designing and instruction about patient care delivery systems in collaboration with the NICU Manager and Nursing Director of MCH.
  4. Plans, coordinates, evaluates and designs orientation tailored to the educational needs of the staff.
  5. Consistently plans and organizes orientation programs for the needs of new hires.
  6. Plans and develops appropriate educational programs for staff.
  7. Insures quality of patient care by developing, monitoring, and evaluating clinical protocols, standards of care and practice, policies and procedures.
  8. Assists in coordinating educational programs
  9. Maintains teaching materials, education tools and files on all programs.
  10. Assists in the communication of revised patient care procedures to the patient care staff and new employee
  11. Coordinates in-service sessions to implement and inform the staff of new products and equipment as necessary
  12. Utilizes new information and/or research findings in developing and updating the content of programs and specific teaching activities
  13. Develops and implements formal and informal education programs and unit specific orientation programs to meet identified needs of nursing staff.
  14. Collaborates with clinical and administrative colleagues for the development and implementation of joint educational projects to meet identified nursing service needs
  15. Assist in developing evidence based clinical policies and procedures in conjunction with the NICU Manager and Director of MCH.
  16. Educates the interdisciplinary team in evidence based protocols and standards of care related to patient populations. 
  17. Functions as a resource person for staff members on complex patients
  18. Assures clinical competencies of nursing staff through educational programs and evaluation processes
  19. Provides oversight of comprehensive primary caregiver education tools
  20. Identifies and communicates learning opportunities available for unit level staff within the hospital and community
  21. Assures responsibility for special projects as assigned
  22. Understands and implements the current TJC, CCS and Title 22 requirements for nursing personnel
  23. Collaborates with the NICU Manager for data entry in the CPQCC and completion of the annual CCS Report.
  24. Reviews Policy and Procedure Manuals to maintain a complete understanding and compliance of hospital regulation and new evidence based information.
  25. Observes hospital and department standard operation procedures in precautions for transmission of infectious diseases and the handling and use of hazardous substances.

Revises the plan of care as indicated by the patient’s response to treatment and evaluates overall plan daily for effectiveness. Performs patient care responsibilities considering needs specific to the standard of care for patient’s age. Receives physician’s orders, ensures transcription is accurate and documents completion. Administers medication utilizing the five rights of medication administration reducing the potential for medication errors. Formulates a teaching plan based upon identified learning needs and evaluates effectiveness of learning; family is included in teaching as appropriate. Assists physicians with examinations, treatments and special procedures and performs services requiring technical and manual skills within scope of practice. Performs treatments and provides services to level of licensure. Treats patients and their families with respect and dignity. Identifies and addresses psychosocial, cultural, ethnic, and religious/spiritual needs of patients and their families. Functions as liaison between administration, patients, physicians, and other healthcare providers. Interacts professionally with patient/family and involves patient/family in the formation of the plan of care. Interprets data about the patient’s status to identify each patient’s age specific needs and provide care needed by the patient group. Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors. Initiates or assists with emergency measures for sudden adverse developments in patients’ condition. Answers telephone, paging system, patients’ call lights, anticipates patients’ needs, and makes rounds of assigned patients and responds as appropriate. Consults other departments as appropriate to provide for an interdisciplinary approach to the patient’s needs. Provides end of shift report to oncoming nurse, narcotics are counted, documentation is complete, and physician orders signed off. Communicates appropriately and clearly to management, co-workers, and physicians. Identifies and addresses psychosocial needs of patients and family; communicates with Social Service/Discharge Planner regarding both in hospital and post discharge needs. Participates in orientation, instruction/training of new personnel. Manages and operates equipment safely and correctly. Knowledge/Skills/Abilities/Expectations Knowledge of medications and their correct administration based on age of the patient and their clinical condition. Basic computer knowledge. Able to organize tasks, develop action plans, set priorities and function under stressful situations. Qualifications Education Postsecondary certificate, diploma, or program graduation from an accredited school of nursing. (Required) Associate’s Degree or Bachelor’s Degree in Nursing. (Preferred) Licenses/Certifications Registered Nurse (RN) – State Licensure and/or Compact State Licensure in the state of practice. (Required upon hire) Basic Life Support (BCLS). (Required upon hire) Advanced Cardiac Life Support (ACLS). (Preferred upon hire) Experience Six (6) months of Medical/Surgical nursing experience in an acute care setting.
