As a LPN (Licensed Practical Nurse) in Personal Care, you will carry out our Mission of Love by ensuring the care of our residents is provided in a safe, caring, timely, and appropriate manner by maintaining the appropriate competencies and skills through continuing education that encompasses all of these areas. This will include assisting our residents in reaching or attaining the highest potential in their physical, mental, and psychosocial well-being.

Our LPN (Licensed Practical Nurse) in Personal Care is responsible for the following:

  • Monitors nursing care by visiting residents routinely to ensure continuity and continual evaluation of care as related to care needs and provider orders. Assesses resident's condition and nursing care needs, assures that medications, treatments, and care are provided as ordered. Maintains appropriate documentation of nursing observations, including follow-up documentation in the medical record in accordance with acceptable standards of practice. Provides direct resident care and transports residents to activities and appointments as time permits. Performs glucometer readings, PVRs and other testing as needed.
  • Reports upon first notice any environmental situation which could pose a threat to the safety or well-being of the Masonic Village Residents. Responds to Fire and Disaster alarms and drills according to procedures. Assists residents to safety during an evacuation.
  • Observes residents and answers call bells in a timely manner to respond to residents' care needs. Reports observations, changes, unusual conditions or reactions to supervisor or appropriate discipline in a timely manner. Takes appropriate action as requested or required by the situation. Notifies family of change in residents' condition or hospitalization according to Masonic Village policy.
  • Performs accurate assessments of resident needs and strengths. Assists in the development of the plan of care as well as in the completion/update of the RASP and other mandated forms as needed and within mandated timelines. Implements the plan of care and oversees care provided by assigned nursing personnel.
  • Communicates effectively and in a non-aggressive manner with residents, family members, visitors and co-workers. Establishes and maintains collaborative communication with providers, other health professionals and Masonic Village departments.

What You Have:

  • Graduation from an accredited school of nursing as a Licensed Practical Nurse.
  • Current LPN licensure by the PA State Board of Nursing.
  • CPR certification (prior to or upon employment).
  • Previous experience in long-term care preferred yet not required.
