POSITION SUMMARY

Our Wellness Nurse (LVN) plays a vital role in ensuring the health and well-being of our residents. In collaboration with the Health and Wellness Director, this role provides high-quality care through health assessments, medication management, and coordination with residents, families, and the care team. Responsibilities include monitoring resident health, addressing changes in condition, staff training and supervision, and promoting a safe and supportive environment that aligns with our mission to provide exceptional care.

KEY RESPONSIBILITIES

Perform resident assessments and reassessments according to policies, monitoring any changes in condition.
Communicate with the Health and Wellness Director about resident needs to ensure consistent quality of services.
When necessary, participate in hands-on care, medication administration, and specific nursing tasks.
Schedule and assign caregiving and med tech shifts to ensure adequate coverage and compliance with staffing guidelines and department allowances.
Supervise, train, and support care team members, promoting teamwork and adherence to care standards.
Assist in the transcription of physicians' orders and the review of new physicians' orders.
Maintain accurate and up-to-date resident records, including care plans, incident reports, and medical documentation.
Coordinate care planning with home health agencies on-site, working within community and company policies.
Address residents’ special needs, requests, and complaints.
Coordinate admissions with the sales team, including supervising move-ins.
Understand the community's care regulations along with state-specific regulations.
Facilitate communication between residents, families, and healthcare providers to ensure seamless care delivery.

Requirements

CANDIDATE QUALIFICATIONS

Education and Certifications:

A valid, incumbent nursing license (LVN) is required.
High School Diploma or equivalent.
An associate degree or higher in nursing is preferred, or a combination of education and experience is required. Additional coursework in management is an asset.
Current First Aid and CPR license or ability to obtain.

Experience, Competencies, and Skills:

At least four (4) years working in geriatric nursing, including experience with medication administration, delivery systems, and pharmacies.
At least one (1) year of relevant supervisory experience, including staff scheduling and management, is preferred.
Experience with memory care is a plus.
Excellent attention to detail and high integrity.
Excellent communication skills and teamwork mindset.
Strong leadership and team management skills.
Outstanding organizational skills.
Positive attitude, empathy, patience, and commitment to treating our residents with dignity and respect.
Ability and willingness to participate in on-call rotation.
Must meet all mandatory health requirements according to state regulations.

Job Description The Medical Coordinator is responsible for delivering high quality medical care and influencing positive outcomes for patients experiencing homelessness in the recuperative care setting. We have many schedules available for this role, including weekends and nights. The pay range for this position is $34.00 - $36.00 per hour, depending on experience . This position also qualifies for $1.50 swing shift differential and $3 graveyard shift differential. The schedule for this position is 7:00am to 3:30pm. Workdays may vary. Responsibilities Client Care: Perform initial intake to ensure that an appropriate care plan is established based on discharge information from the hospital or primary care notes Assess health status of patient upon arrival by conducting head to toe assessment and checking vitals Determine the patient’s insurance status, make follow up appointments with patient’s providers and reviewing medication list Establish and maintain a care plan Set weekly goals for patient to complete to assist with medical or social reintegration, verify goal completion, and assess medical needs. Conduct morning, afternoon, and evening checks for all assigned clients to observe wellbeing, ensuring there is no change in medical conditions since hospital discharge. Link clients to medical services when necessary (insurance, PCP, specialists, referrals). Coordinate home health visits (if needed) and follow-up on medical appointments. Promote health literacy by providing patient education with regards to clients’ illnesses, medications, preventative measures, treatment management and overall general health. Arrange transportation for clients to and from medical appointments, as needed Report emergency situations to Supervisor/Manager and document incident Seek proper level of care to assess and treat medical complication Initiate medical response system when appropriate Support patient recuperation by providing necessary items and space needed to rehabilitate and assisting with the upkeep of the facility. Administrative/Record Keeping: Enter patient information and case notes into EHR and other documentation systems.
