Job Summary/Overall Objectives 

The Licensed Practical Nurse functions as a member of the health-care team by exercising sound nursing judgment.  The LPN provides direct and indirect patient care for an assigned group of patients under the direction and supervision of the RN and according to the plan of care derived from physician's orders and Registered Nurse assessment.  Nursing Care includes physical care, emotional support, and patient and family education. 

Essential Job Functions

  • Delivers direct and indirect patient care in accordance with the Pennsylvania Practical Nurse Law, hospital policies, procedures, protocols and other professional standards of care.
    • Assists the RN in collecting patient health care data on admission by completing a focused assessment in an ongoing systematic manner, focusing on the physical, psychosocial, age, cultural and spiritual needs of the patient and which contributes to the comprehensive assessment by the RN.
    • Works directly with the RN to implement an individualized, age-specific plan of care and evaluate its effectiveness.
    • Assists the RN in reassessment of the patient's condition and takes appropriate action as indicated or by unit specific policy.
    • Involves the patient/family and other health care providers when appropriate.
    • Completes patient care related activities within scheduled time periods.
    • Complies with completeness, accuracy and timeliness of documentation, utilizing the electronic medical record.
  • Observes, records and reports to the appropriate professional the symptoms, reaction and changes including:
    • General physical and mental condition of patients, and signs and symptoms which may be indicative of untoward changes; and stresses in human relationships between patients, between patients and personnel, and between patients and their families and visitors.
  • Performs nursing functions as defined in Pennsylvania regulations, (where applicable by location and department need) such as:
    • Administration of medications, immunizations, and therapeutic treatments prescribed by the physician;
    • Preparation and care of patients receiving specialized treatments administered by the physician or the professional nurse.
  • Assists with the rehabilitation of patients according to the medical care plan, being aware of and encouraging the interests and special aptitudes of patients.
    • Encouraging patients to help themselves within their own capabilities in performing activities of daily living.
    • Knowledge and application of the principles of prevention of deformities, the normal range of motion, body mechanics, and body alignment.
  • Assists in providing education for the provision and coordination of patient care activities that promote and maintain health, foster self-care and improve outcomes.
    • Assists the RN in assessing the patient's readiness to learn, identifying educational barriers and adapting the process to meet the needs of the patient.
    • Ensures education about safely and effectively using medication, pain and managing pain, available resources, how to obtain further care, and information about responsibilities in their care.
    • Acquires and distributes educational resources tailored to patient needs.
    • Provides and reviews, upon request of the RN, written discharge instructions that demonstrate an inter-relationship between education, discharge planning and continuity of care.
  • Maintains a collaborative relationship with physicians and other health care providers to achieve desired patient outcomes and assure a continuous flow of patient care.
    • Communicates pertinent information for interdisciplinary patient care conferences, shift reports and internal and external transfers.
    • Reports changes in patient's condition to the RN, including the need to reassess patient or change the plan of care; confers with the RN when there is a need to contact the physician.
  • Participates in the unit performance improvement activities designed to enhance the quality of patient care and customer service.
    • Assists in data collection process as requested.
    • Recommends modifications for performance improvement.
  • Demonstrates leadership by utilizing the concepts inherent to the practice of the professional nurse.
    • Demonstrates an ability to solve problems independently and seeks assistance from immediate supervisor when appropriate.
    • Delegates patient care appropriately.
    • Maintains accountability for actions taken.
    • Effectively functions as a resource person.
    • Acts as a patient advocate.
    • Performs effectively in the role of preceptor/mentor to new hires and/or students.
    • Assists in maintaining/decreasing organizational/department costs.
  • Assists in the department's compliance with State, TJC, OSHA and other regulatory agencies.
    • Participates in continuous survey readiness for inspections and surveys.
    • Participates in the non-punitive medication reporting system.
    • Ensures accountability for narcotics per regulatory/organizational policies.
    • Adheres to the infection control process, including standard precautions, to reduce the risk of acquiring or transmitting infections.
  • Provides for patient safety in compliance with hospital and nursing department policies.
    • Utilizes two forms of patient identification before implementing any form of patient care.
    • Involves the patient and/or family in patient safety standards and processes.
    • Minimizes the use of patient restraints and, if patient's condition warrants, monitors per individual assessed need and documents per policy.
    • Assumes appropriate code team role during code situations, including internal and external disasters.
  • Other duties as assigned.

Competency and Experience [Knowledge, Skills and Abilities]

Required Qualifications/Experience

  • Graduate of an accredited School of Practical Nursing
  • Zero (0) to two (2) years of experience in behavioral health
  • Strong leadership ability, good organizational skills, independent and critical thinking skills, sound judgment, and knowledge of legal aspects and liability of nursing practice.
  • Strong ability to communicate complex and/or controversial topics and concepts to a wide and diverse audience.

Preferred Qualifications/Experience

  • Previous LPN experience
  • Experience in Healthcare setting.

License, Certification & Clearances

  • Current licensure to practice as a Licensed Practical Nurse in the State of Pennsylvania
  • Current Basic Life Support (BLS) approved curriculum of the American Heart Association (CPR and AED) program.
  • Act 33 with renewal
  • Act 34 with renewal
  • Act 73 FBI Clearance

Position Type/Expected Hours of Work

  • Incumbent will be scheduled based on operational need (rotate shifts, standby, on-call, etc.).
  • Travel may be expected locally between System locations.

LPN Patient Care Advocate II

Job Summary The Patient Care Advocate II (LPN/LVN) is a licensed, field-based member of WellBe’s interdisciplinary geriatric care team who delivers white-glove, high-quality, in-home care to frail, poly-chronic, elderly Medicare Advantage patients in the comfort of their own homes. This role combines patient-advocacy, care-coordination, and quality (HEDIS) responsibilities with clinical and telehealth visit-facilitation responsibilities. Practicing within the LPN/LVN scope of practice and at the direction of the assigned Provider, the Patient Care Advocate II conducts in-person patient visits, facilitates in-home telehealth consultations with an APP or Physician, captures and documents clinical data, supports patient education, and serves as the local-market connector for patient support services across the WellBe platform. The role models the WellBe mission and values, works closely with the provider and social-work teams, and functions as part of an integrated, interdisciplinary care team with no direct-report responsibility. The incumbent follows the rules and regulations of state and federal regulatory agencies and other certifying bodies for their region. Daily, local travel to meet patients and engage the community is an essential part of the role. Job Description Clinical Care & Visit Facilitation (LPN scope, at the direction of the assigned Provider) Facilitate in-home telehealth visits between the WellBe APP or Physician and the patient, including initial patient assessments, urgent visits and ongoing follow-up visits as appropriate to patient need. Check and record health vitals — including weight, blood pressure, heart rate, lung sounds, and blood-sugar levels — and document health changes observed during the visit or reported by the patient. Complete and document a thorough home-safety inspection during patient visits. Provide direct patient-care support and patient education at the direction of the assigned Provider, within the LPN scope of practice. Complete minimally-invasive patient screenings and point-of-care testing (POCT), and clearly explain POCT to geriatric patients. Subject to applicable state laws and restrictions, training, and company protocols, safely and competently perform venipuncture and capillary blood collection for geriatric patients in home settings. Support the Comprehensive Initial Visits, Follow-up Visits, and Urgent Visits. Complete drop-in visits for the unengaged and disengaged patients and support open HEDIS . Conduct check-in visits/calls to our lower acuity patients, enhancing our “call us first” model. Escalate patient clinical concerns to the APP, Community Operations Director (COD), or Community Medical Director (CMD). Patient Advocacy & Care Coordination Develop rapport and deliver exceptional, compassionate in-home service; provide positive customer service and treat all patients and staff with respect. Act as a community resource for the patient and work to engage patients who are not currently engaged in care. Schedule patient appointments with the WellBe team and coordinate specialist appointments and diagnostic tests. Answer patients’ clinical questions strictly within the boundaries of the SOP. Explain WellBe processes and the care journey. Review patient-care interventions and assess patient needs for transitions of care, end-of-life discussions, palliative-care interventions, and medication adherence. Quality, Documentation & Team Collaboration Support the APP in ongoing panel management and quality-of-care efforts; utilize HEDIS tracking tools to meet and improve HEDIS / STARS performance and close gaps, including medication adherence. Contribute complete, accurate, and timely data to patients’ records and ensure all required clinical and non-clinical documentation is in the EMR; complete and submit all documentation daily. Collaborate with the interdisciplinary team and participate in regularly scheduled team and Interdisciplinary Team (IDT) meetings, providing additional information on patient and family interactions. Build relationships with local community providers and caregivers to maintain WellBe’s presence and reputation in local markets. Attend all required trainings, including equipment / device training; complete ongoing education as additional skills and protocols are added. Operate and drive their vehicle while conducting Company business in a safe and professional manner, always exercising due regard for self and other motorists. Job Requirements Compensation $30.00 - $38.00/hr Educational / License Requirements High-school diploma / GED Active, unrestricted Licensed Practical Nurse (LPN) / Licensed Vocational Nurse (LVN) license, with residency, in the state of service. Minimum of two (2) years as an LPN/LVN in a direct, patient-facing care-delivery role. Current BLS / CPR certification for healthcare providers, or willingness to obtain. Demonstrated ability to perform high-quality venipuncture and capillary draws on geriatric patients in home settings (where required by market and consistent with state law). Valid driver’s license with an acceptable driving record, and proof of an active auto-insurance policy. Prior telehealth experience preferred. Previous experience in home care, hospice, palliative care, or geriatrics strongly preferred; other complex patient-health-care experience also beneficial. Required Skills and Abilities Strong communication skills, with the ability to effectively interact with patients, families, and remote healthcare professionals. Ability to work independently and make sound decisions while adhering to established protocols, guidelines, and the LPN/LVN scope of practice. Excellent organizational and time-management skills to balance in-person visits, telehealth facilitation, and administrative tasks. Familiarity with electronic medical records (EMR) systems and strong computer skills, including Word, Excel, and PowerPoint. Understanding of risk assessment, quality measures, and full-risk / value-based care. High level of professionalism and a strong geriatric customer-service focus. Experience working with culturally diverse and underserved populations.
