Under the supervision of the Ambulatory Administrator or Director, Manager and Medical Director, utilizes the Nursing Process to provide care, coordination of care and patient education to a group of patients, using the medical home mode. Assists providers in the provision of quality healthcare. Assignments are performed appropriately to age and developmental level of patients being served. Provides excellent customer service in a team-based environment.
Responsibilities
Assists the clinician in delivering quality patient care includingmeasuring vital signs, obtaining history, initiating EMRprogress note and updating allergy, medication and problemlists. Performs or assists with dressing changes, pulseoximetry, respiratory treatments, point of care testing, vision &hearing screens, and other required procedures as indicated.Facilitates scheduling and follow up for patients referred forconsults and procedures. Administers vaccines, medications and treatments as orderedby provider and within scope of practice. If in a clinic thatserves pediatric patients, ensures vaccines are stocked andin compliance with Vaccines for Children/Florida SHOTSprogram. Vaccines For Children responsibility applies inclinics that participate in the program. Plans and implements age-appropriate care based onassessment, diagnosis, standard of care, medical treatmentplan and progress toward desired outcome. Assessesindividual physical, psychosocial and developmental needs ofpatients. Identifies patient/family education needs for growth,development, safety, nutrition, and wellness; coordinates ordelivers training and evaluates the effectiveness of theteaching plan. Accurately and efficiently documents findings, plans andinterventions in the electronic medical record. Evaluates theeffectiveness of care and documents subsequent changes toplan in the patient medical record. Acquires and maintains knowledge and skills in nursingpractice to contribute to the professional development of self,peers, colleagues, and others. Anticipates and performs preliminary specimen collection andCLIA waived lab testing as indicated (e.g. Urinalysis, rapidstrep, hemoglobin, lead, etc.). Provides phone triage and coordination of scheduling urgentappointments as indicated.
Qualifications
Experience Requirements:
1 year - Ambulatory Care setting Education: Education - Associates required, Bachelors preferred
Certification/License:
Certifications: Basic Life Support (BLS) - Required at time of hire Registered Nurse License (FL) - Required at time of hire
